不同照射野三维适形放疗在局部晚期非小细胞肺癌中的应用

目的：分析不同照射野三维适形放疗在局部晚期非小细胞肺癌中的应用价值。方法：回顾性分析2005年1月至2008年6月收治的132例局部晚期非小细胞肺癌患者资料,均行三维适形放疗,依据照射野的不同,分成3野组与5野组,常规分割进行照射,5次／周,DT60Gy-66Gy／30f-33f。观察两组患者近期疗效、V30、V20、V15、V10、V5等参数以及放射性肺炎的发生,并进行比较。结果：两组患者之间近期疗效（CR＋PR）、V20、放射性肺炎的发生均无差异（P〉0．05）;3野组V30、V15、V10高,而5野组V5较高,V30、V10、V5具有非常显著差异（P〈0．01）,V15具有显著差异（P〈0．05）。结论：采用3野或5野进行照射,效果相似。

Radiation program vealuation on different fields in 3DCRT for patients with locally ad- vanced non- small- cell lung cancer

Objective：To analyse the value of different fields in the three dimensional conformal radiotherapy （3 DCRT） for patients with locally advanced non -small -cell lung cancer（ LANSCLC ）. Methods： Data of 132 patients with LANSCLC were analysed retrospectively. All patients were treated by 3DCRT. They were divided into three fields group and five fields group. 3DCRT was given by five times a week and 2Gy per fraction with the total doses from 60Gy to 66 Gy,which were given by 30 to 33 fractions. The response rates,V30,V20,V15 ,V10,V5 and radiation pneumonitis were observed. Results： There was no difference between the two groups on the response rates （ CR ＋ PR ）, V20 and radiation pneumonitis （ P 〉 0.05 ）, there were significant difference on V30 （ P 〈0.01 ）, V15（P 〈0.05） ,V10（P 〈0.01 ） ,which were higher in the three fields group than that in the five fields group,andV5 （P 〈0.01 ） ,which was higher in the five fields group. Conclusion ： Radiotherapy could be well utilized by three fields or five fields in 3DCRT for LANSCLC ,the effects are similar.
