调强放疗联合TP方案化疗治疗局部中晚期食管鳞癌的临床研究

目的 观察调强放射治疗（IMRT）联合TP方案化疗治疗局部中晚期食管鳞癌的疗效.方法 食管鳞癌患者74例,分为观察组和对照组,对照组采用单纯IMRT,观察组采用IMRT联合TP方案化疗.结果 所有患者均完成放化疗计划,观察组客观缓解率（ORR）为81.1％,临床受益率（CBR）为91.9％.对照组ORR为67.6％,CBR为73.0％.2组间ORR无显著差异（P＞0.05）,但观察组CBR显著高于对照组（P＜0.05）.观察组2年生存率为70.3％,对照组2年生存率为59.5％,组间无显著差异（P＞0.05）.2组患者不良反应主要是放射性食管炎和骨髓抑制.结论 IMRT联合TP方案化疗治疗局部中晚期食管鳞癌具有一定的优势,且不良反应无明显增加.

Clinical Research of Intensity Modulated Radiotherapy Concomitant with TP Protocol on Local Middle and Advanced Esophageal Squamous Carcinoma

OBJECTIVE To observe the clinical efficacy of intensity modulated radiotherapy （IMRT） concomitant with taxotere and platinol （TP） protocol on local middle and advanced esophageal squamous carcinoma. METHODS A total of 74 patients with esophageal squamous carcinoma were randomly divided into control group treated with single IMRT and observation group with IMRT concomitant with TP protocol, 37 cases for each. RESULTS All patients finished the chemotherapeutic protocols. The objective response rate （ORR） and clinical benefit rate （CBR） were 81.1% and 91.9% in observation while were 67.6% and 73.0% in control group, respectively. Though the there was no significant difference in ORR （P〉0.05）, the CBR was evidently higher in observation group than in control group （P〈0.05）. The 2-year survival rate was 70.3% in observation group and 59.5% in control group respectively, and the difference was not significant （P〉0.05）. The adverse responses were mainly radioactive esophagitis and myelosuppression in both groups. CONCLUSION IMRT concomitant with TP protocol have certain advantages without obvious increase of adverse responses in the treatment of local middle and advanced esophageal squamous carcinoma.
