两种不同咬合板治疗颞下颌关节紊乱病的临床效果

目的探讨两种不同咬合板治疗颞下颌关节紊乱病的临床效果。方法选取鹤壁市人民医院2017年1月至2018年4月收治的颞下颌关节紊乱病患者42例,按随机数字表法分为A、B 2组:A组21例予以稳定性咬合板治疗,B组21例予以Twin-block咬合板治疗。比较2组治疗效果、不同时间点疼痛、关节弹响及张口度变化情况。结果 2组治疗1、3、6个月后VAS评分较治疗前均显著降低,且A组较同时间点B组降低更为明显,差异均有统计学意义（均P<0.05）;2组治疗1、3、6个月后张口度均较同组治疗前显著增大,且A组较同时间点B组增大更为明显,差异均有统计学意义（均P<0.05）;2组治疗1、3、6个月后关节弹响声均较同组治疗前显著降低,且B组较同时间点A组降低更为明显,差异均有统计学意义（均P<0.05）。结论对颞下颌关节紊乱病患者采用稳定性咬合板和Twin-block咬合板治疗均有较好的治疗效果,稳定性咬合板在缓解关节疼痛和张口受限方面优于Twin-block咬合板,但Twin-block咬合板在改善关节弹响方面效果更显著。

Effects of Two Types of Occlusal Plates on Temporomandibular Joint Disorders

Objective To investigate the effects of two types of occlusal plates on temporomandibular joint disorders.Methods Forty-two patients with temporomandibular joint disorders admitted to Hebi People’s Hospital from January 2017 to April 2018 were randomly treated with either stable occlusal plate(group A,n=21)or twin-block appliance(group B,n=21).The therapeutic effect,pain,joint clicking and mouth opening were compared between the two groups.Results After treatment for 1,3 and 6 months,VAS scores and joint clicking sound decreased and mouth opening degree increased in both groups(P<0.05).Furthermore,the improvement in VAS scores and mouth opening degree in group A was more obvious than that in group B(P<0.05).However,the decrease in joint clicking sound in group B was more significant than that in group A(P<0.05).Conclusion Both stable occlusal plate and twin-block appliance are effective for temporomandibular joint disorders.Stable occlusal plate is superior to twin-block appliance for relieving joint pain and mouth opening restriction,but inferior to twin-block appliance for improving joint clicking.
