肺大泡的外科治疗

目的　探讨肺大泡外科治疗方法和治疗效果。方法　以 1999年 1月至 2 0 0 2年 5月在中日友好医院治疗的 4 7例肺大泡患者为研究对象 ,采用胸腔镜或开胸手术方法 ,行肺大泡切除术。结果　 2 3例患者在胸腔镜或胸腔镜辅助下小切口行肺大泡切除术 ,2 3例选择后外侧切口肺大泡切除术 ,均痊愈出院 ,1例双侧肺大泡行胸骨正中径路同期手术 ,于术后 19d死于肺部感染、肾功能衰竭、感染中毒性休克。结论　对于肺大泡病例 ,选择胸腔镜还是开胸手术 ,关键在于肺大泡的大小及位置 ,与数量多少无明显关系。对于双侧肺大泡的某些患者分期手术可能更为合适

Pneumotoceles' surgical treatment

Objective Research on lung bubble surgical treatment and its effect.Methods This study is based on 47 cases of lung bubble patients,who received treatment of thoracoscopy or thoracotomy to resect lung soaks in Sino-Japanese Friendship Hospital during January 1999 to May 2002.Results 23 cases received the bubble resection thoracoscopically,or received a minor subaxillary incision with the aid of thoracoscope to resect them;23 cases received posterolateral incision to resect bubbles,all the above cases were fully recovered.One case of bilateral bubbles received mid-sternal incision approach during the same period,but died of lung infection,renal failure,and infectious toxic shock on the 19th day after operation.Conclusion For lung bubble cases,regardless of the methods mentioned above,the crucial point is size and position of the lung bubbles,there is nothing much to do with the quantities of the bubbles.For some patients suffered from big bubbles in both lungs,multi-stage operations may be more suitable.
