二维超声心动图与多平面经食管三维超声心动图测量左室收缩功能的对比研究

摘    要:应用多平面经食管三维超声心动图技术(3DE)对30例冠心病患者(A组)及7例非冠心病患者(B组)的左室容量和射血分数等心功能指标进行了分析,并与二维超声心动图(2DE)及心导管左室造影(LVA)的测值进行对比,结果显示:①在B组患者中,2DE及3DE所测左室容量及射血分数均与左室造影结果高度相关(P<0.01);在A组患者中,2DE所测各项指标的相关性明显下降,而3DE仍与左室造影相应测值呈高度相关(P<0.01);②在B组患者中,2DE及3DE与左室造影所测EF无显著性差异(P>0.05);在A组患者中,2DE所测EF高于左室造影结果(P<0.05),而3DE所测EF与左室造影结果无显著差异(P>0.05);③A组患者的ESV显著高于B组,而EF则小于B组(P<0.05)。表明冠心病的存在使2DE左室功能测量的准确性显著下降,而对3DE左室功能测量的准确性则无明显影响;同时,与非冠心病组相比,冠心病患者的ESV明显增加而EF则显著下降。

Abstract:To evaluate the accuracy of transesophageal three-dimensional echocardiography (3DE) in measuring left ventricular volume and ejection fraction in patients with coronary artery disease (CAD), 30 patients with CAD and 7 cases without CAD for controls were examined by 3DE, two-dimensional echocardiography (2DE) and left ventriculography(LVA) to determine their left ventricular function. The results showed that: 1. in 7 patients without CAD, the parameters determined by 2DE and 3DE both correlated highly with LVA; whereas in 30 patients with CAD, the correlations of 2DEdeclined obviously, but there were still high correlations of EDV, ESV, SV and EF measured by 3DE and LVA. 2. the ESV increased and EF decreased significantly (p<0. 05) in CAD patients compared with those of patients without CAD.
