细胞凋亡和P53、PCNA表达在宫颈癌的研究

摘    要:目的 :探讨细胞凋亡、P5 3蛋白和PCNA表达与宫颈癌发病的关系及与其生物学行为的关系。方法 :利用流式细胞仪PI法 ,测定正常宫颈组织 18例、宫颈上皮内瘤变 (CIN) 18例和宫颈癌 2 3例细胞凋亡指数 (AI) ,采有SP免疫组织化学染色方法检测各组宫颈上皮组织中P5 3蛋白和PCNA表达。结果 :正常宫颈组织AI值明显低于CIN组和宫颈癌组 (P <0 0 1) ,而宫颈癌组AI值明显低于CIN组 (P <0 0 5 )。突变P5 3蛋白和PCNA表达随宫颈各级病变的进展逐渐升高 ,二者在正常组织、CIN、宫颈癌三组中阳性表达率分别为 5 6 %、16 7%、4 7 8%和 5 6 %、6 1 2 %、78 3%。P5 3蛋白表达与宫颈癌组织分化级别、淋巴结转移无关 (P >0 0 5 ) ,而与宫颈癌临床分期有相关性 ;PCNA与宫颈癌临床期别、淋巴结转移无关 (P >0 0 5 ) ,而与宫颈癌组织分化级别有相关性 (P <0 0 1)。结论 :细胞凋亡和增殖的失衡在宫颈癌发生、发展过程中具有重要作用 ,野生型P5 3基因突变可能是细胞凋亡受抑的一个重要因素 ,P5 3及PCNA是判断宫颈癌的恶性程度和预后很重要的分子生物学指标。

Abstract:Objective:To evaluate apoptosis,the expression of P53,PCNA and its relationship to occurrence and histopathological features of cervical cancer. Methods:Flow-cytometry was introduced in the study of apoptosis, and the expression of P53 and PCNA were evaluated by immunohistochemistry in 23 specimens of cervical cancer, 18 specimens of normal cervical organization and 18 specimens of cervical intraepithelial(CIN).Results:Apoptosis Index(AI) in normal group was lower than that in CIN group (P<0.01), and AI in cervical cancer group was lower than that in CIN group (P<0.05). The positive rate of P53 in three groups were 5.6%,16.7% and 47.8%. The expression of P53 was related to clinical staging (P<0.01). The expression of PCNA was associated with histopathological grading (P<0.01).Conclusion:Imbalance between apoptosis and proliferation is important to cause and improve cervical cancer. Mutation of Wild p53 gene may be an important factor on restraining apoptosis of carcinomatous cells. P53 and PCNA may be a hopeful marker for predicting the metastasis degree of cervical cancer and the prognosis of those patients.
