血液透析患者脂代谢与甲状旁腺功能的相关研究

目的探讨维持性血液透析患者的血脂代谢异常与继发性甲状旁腺功能亢进的关系。方法对70例行血液透析的患者检测总胆固醇（TC）、甘油三酯（TG）、高密度脂蛋白胆固醇（HDL-C）、低密度脂蛋白胆固醇（LDL—C）、甲状旁腺激素（PTH）。随机选取80例无肾脏病的体检对象为对照组。结果血液透析组PTH、TG、LDL—C及LDL-C／HDL-C比值较正常组明显升高（P〈0．05），HDL—C明显下降（P〈0．05），Tc在两组间差异无统计学意义（P〉0．05）。血液透析组PTH水平与TC、TG、HDL-C、LDL-C无明显相关（P〉0．05），与TG、LDL-C／HDL—C比值成正相关（P〈0．05）。结论脂代谢异常及继发性甲状旁腺功能亢进是维持性血液透析患者常见的并发症，高P，rH水平可能与脂代谢异常有一定的相关性。

Relationship between Lipid Metabolism and Secondary Hyperpara Thyroidism in Maintenance Hemodialysis Patients

Objective To investigate the relationship between abnormal lipid metabolism and secondary hyperpara thyroidism （PTH） in maintenance hemodialysis patients. Methods Blood levels of TC,TG, HDL-C, LDL-C, PTH were measured in 70 maintenance hemodialysis patients and 80 subjects without renal disease（ control group）. Results Compared with the controls,hemodialysis patients with higher levels of PTH, TG, LDL-C and LDL-C/HDL-C （ P 〈 0.05 ）, but lower level of HDL-C （ P 〈 0.05 ）. Linear correlation analysis showed that blood PTH levels had no significant correlation with blood levels of TC, HDL-C, LDL-C, but had a positive correlation with blood levels of TG, LDL-C/HDL-C. Conclusion Dyslipidemia and secondary hyperpara thy- roidism are the frequent complications in maintenance hemodialysis patients. The level of PTH may have positive correlation with the dyslipidemia.
