重症急性胆管炎术后经双腔T管行胆道冲洗的临床观察

４０例行手术治疗的重症急性胆管炎病人随机分为二组：冲洗组２０例，术中放置双腔Ｔ管，术后用抗生素溶液持续冲洗胆道７天，对照组２０天，术中置普通Ｔ管，术后持续引流胆道。结果发现，冲洗组术后并发症发生率，死亡率及胆汁细菌培养阳性率均明显低于对照组，术后体温，白细胞计数和血清胆红素降至正常的时限亦短于对照组。

CLINICAL OBSERVATIONS ON BILIARY LAVAGE AFTER OPERATIONS FOR ACUTE CHOLANGITIS OF SEVERE TYPE

Forty patients with acute cholangitis of severe type (ACST)were randomly divided in to two groups. For binarylavage group (n=20),double channel T-tules were inserted into the binary tract at operations. The binary lavage using the doublechannel T-tube begun immediatly and ended at 7th day after the operations. For contral group (n=20),common T-tubes were inserted at operation for drainaging the bile after operations. The results showed that in the binary lavage group the morbidity,mortality and residual stone rate decreased significantly compared with that of the contral group. The temperature,white blood count,serum bilirubin and positive rate of bile bacteria culture lowered more rapidly than that of the contral group. It is suggested that thebiliary lavage using double channel T-tube is a practical,safe,effective method in treating the postoperative patients with ACST.
