B超四腔心切面在筛查胎儿先天性心脏畸形中的价值

目的：分析四腔心切面在筛查胎儿心脏畸形中的价值。方法：对、971例胎儿，其中259例为高危胎儿，用心脏四腔切面筛查的方法进行检测，以引产后尸解或出生后作为对照标准。结果：终止妊娠胎儿尸解和出生后证实有先天性心脏病的36例，其中有33例用四腔心切面筛查有先天性心脏畸形，占91．67％，3例四腔心切面所见基本正常，但存在先天性心脏畸形，占8．33％。四腔心切面筛查先天性胎心畸形的敏感性为86．84％、特异性为100％、准确性为99．49％。结论：四腔心切面在产前超声筛查新生儿先天性心脏畸形方面具有重要作用和临床意义。

The Clinical Value of Four-Chamber View on the Fetal Congenital Heart Disease Screening

Objective: To evaluate the clinical value of the four-chamber view antenatally in diagnosis of fetal congenital cardiac abnormalities. Methods: 971 cases including 259 high risk fetuses were scanned by four-chamber view. The autopsy of fetal specimens after pregnancy termination or postnatal monitoring were used as reference-standard. Results: 36 cases could be diagnosed as congenital heart disease(CHD) according to the autopsy of fetal specimens after pregnancy termination and neonatal screening. Scanned by four- chamber view, 33 cases were identified to be congenital cardiac abnormalities which was 91.67%, while 3 cases were appeared approximately normal besides existing congenital cardiac abnormalities which was 8.33%. The sensitivity, specificity and accuracy of the four-chamber view for screening fetal congenital cardiac abnormalities were 86.84%, 100% and 99.49 %, respectively. Conclusion: It shows important clinical value of the four-chamber view on the prenatal ultrasound monitoring for neonatal congenital heart abnormalities.
