实时荧光定量PCR法检测鼻咽癌患者血浆游离EB病毒DNA的临床应用

摘    要:目的 了解本地区鼻咽癌(NPC)患者血浆游离EB病毒DNA(EBV—DNA)的检出状况,探讨其诊断价值和临床意义。方法 采用实时荧光定量聚合酶链反应(FQ-PCR)技术对110例鼻咽癌患者和91例其他恶性肿瘤患者(对照组)进行血浆EBV~DNA检测。结果 NPC患者EBV-DNA阳性率为56.4%(62/110),对照组阳性率为8.8%(8/91),经Х^2检验,Х^2=32.36,P〈0.01。男性NPC患者外周血浆EBV-DNA的阳性率为64.4%(47/73),女性患者阳性率为40.5%(15/37),经Х^2检验,二者差异有统计学意义(Х^2=5.67,P〈0.05)。62例血浆EBV-DNA阳性的NPC患者的拷贝数与8例血浆EBV—DNA阳性的其他恶性肿瘤患者的拷贝数比较,差异无统计学意义(经Wilcoxon非参数秩和检验,uc=1.57,P〉0.05)。结论 FQ-PCR检测血浆EBV-DNA对NPC辅助诊断有很高的临床实用价值,男性NPC患者对EBV的免疫力低于女性,更易受侵,其原因与机制有待进一步研究。

Abstract:Objective To investigate the detection status of of plasma free Epstein-Barr virus (EBV) DNA of nasopharyngeal carcinoma (NPC), and explore its diagnostic value and clinical significance with real time fluorescent quantitative polymerase chain reaction in this district and its diagnostic value. Methods Plasma EBV-DNA was detected with real time fluorescent quantitative polymerase chain reaction (FQ-PCR) assay for 110 cases with NPC (NPC group) and 91 cases with other malignant tumors (control group) . Results The positive rate of plasma EBVDNA was 56.4% (62/110) and 8.8% (8/91) in NPC group and control group respectively (Х^2 =32.36, P〈0.01), 64.4% (47/73) and 40.5%(15/37) in male NPC group and female NPC group respectively (Х^2 =5.67,P〈0.05). There was no significant difference of EBV-DNA copies between all 62 positive cases in NPC group and all 8 positive cases in control group (uc= 1.57,P〉0.05). Conclusion The plasma EBV-DNA detection with FQ-PCR is of practical value in diagnosis of NPC.
