噻托溴铵在慢性阻塞性肺疾病大鼠模型气道炎症和重塑中的保护作用

摘    要:目的观察噻托溴铵早期干预对慢性阻塞性肺疾病(COPD)大鼠模型气道炎症和重塑的影响,并探讨其可能机制。方法Wistar大鼠随机分为3组:健康对照组(A组),COPD模型组(B组),噻托溴铵干预组(C组),采用熏烟并气管内注入脂多糖的方法建立COPD模型,C组于早期给予噻托溴铵雾化干预。进行支气管肺泡灌洗液(BALF)中细胞分类计数;观察肺组织病理形态学变化;图像分析测量小气道管壁、平滑肌和胶原厚度。结果B组基本符合人类COPD病理变化,外周支气管平滑肌层和细胞外基质胶原较A组明显增厚,BALF中炎症细胞总数较A组明显增多,C组则较B组减轻。结论噻托溴铵对COPD气道炎症和重塑有抑制作用。

Abstract:Objective To evaluate the effects of tiotropium bromide on the airway inflammation and remodeling in COPD rats. Methods Wistar rats were randomly divided into three groups: normal control group(group A), COPD group(group B), tiotropium bromide treated group(group C).The COPD rat model was established by intratracheal instillation of lipopolysaccharide twice and exposure to cigarette smoking daily. Group C received inhalation of tiotropium bromide since the eighth day. Total and differential cell counts in bronchoalveolar lavage fluid (BALF) were observed; and the pathomorphological changes in the lung were analyzed. The thickness of the airway wall, smooth muscles and collagen in the bronchial wall were measured. Results The changes of pathology in rat COPD model were similar to those of human COPD. Compared with the group A, there was a significant increase in the airway wall, smooth muscles and collagen thickness of the group B. The total inflammatory cells of the group B in BALF increased in comparison with group A. Those of group C decreased significantly statistically. Conclusion Tiotropium bromide inhalation may inhibit airway inflammation and airway remodeling in COPD rat.
