临床输血管理薄弱环节及对策

依据《医疗机构临床用血管理办法(试行)》、《临床输血技术规范》及《浙江省医疗机构临床用血管理考核细则》等法规规章,从输血审批、输血申请、治疗同意书、输血前检查、样本采集与送检、交叉配血、血液交接、输血监控、不良反应监测和临床用血计划等十个方面,对本院2009年10月至2010年9月1120份归档输血病历进行检查考核,从输血管理的角度对问题比较集中的输血治疗同意书、输血申请单、输血前九项检查和输血适应症等几个方面进行现象、原因分析并提出对策。

Weak process and countermeasures on clinical transfusion management

According to clinical transfusion management in medical institute,clinical transfusion technical regulation and medical institutes clinical transfusion management details in Zhejiang province,1120 cases of transfusion medical records between Oct,2009 to Oct.2010 have been checked in aspects of approval,application,consent,pre-transfusion test,specimen collection and delivery,cross matching,blood handing over,quality control during transfusion,adverse events preventing and plan for transfusion.Main problems Focusing on the transfusion letter of consent,transfusion application,9 tests of pre-transfusion and transfusion indication,their causes and recommendations also have been analyzed.
