P物质、血管活性肠肽在OSAHS患者舌根组织中的表达及意义

摘    要:目的探讨P物质及血管活性肠肽在阻塞性睡眠呼吸暂停低通气综合征(OSAHS)舌根平面阻塞中的意义。方法通过免疫组织化学染色对49例OSAHS患者的舌根组织的P物质及血管活性肠肽的表达进行分析。结果OSAHS患者舌根组织中的粘膜下层、小血管周围、腺泡及腺导管周围均有多量P物质及血管活性肠肽表达。P物质在有舌根肥厚组阳性表达率为80.6%(25/31),在无舌根肥厚组的阳性表达率为33.3%(6/18),经统计学处理χ2=10.97,P=0.000?9;血管活性肠肽在有舌根肥厚组的阳性表达率为90.3%(28/31),在无舌根肥厚组的阳性表达率为55.6%(10/18),χ2=12.296?9,P=0.000?5,差异有统计学意义。结论OSAHS患者舌根组织中有多量P物质及血管活性肠肽表达,它们可引起局部组织肿胀并参与神经源性炎症,进而引起舌根肥厚和塌陷。

Abstract:Objective To explore the effect of substances P(SP) and vasoactive intestinal peptide in the tongue base tissue on retrolingual obstruction in patients with obstructive sleep apnea-hypopnea syndrome (OSAHS). Methods Expressions of SP and vasoactive intestinal peptide in tongue base tissues of 49 patients with OSAHS were analyzed by immunohistochemical staining. Results There were a lot of expressions of SP and vasoactive intestinal peptide in the tongue base tissues of OSAHS patients, which mainly lay in the submucosa tissue and small blood vessels around the gland acini and ducts. The positive expression rate of SP was 80.6% (25/31) and 33.3% (6/18) in those with hypertrophic tongue base and those without hypertrophic tongue base respectively ( χ~2 - 10.97, P - 0.000 9). The positive expression rate of vasoactive intestinal peptide was 90.3% (28/31) and 55.6% (10/18) in the above mentioned two groups respectively (χ~2 = 12.2969, P = 0.0005). Conclusions There are a lot of expressions of SP and vasoactive intestinal peptide in the tongue base tissues of OSAHS patients, which causes local tissue swelling and participates in neurogenic inflammation, and then causes tongue base hypertrophy and collapse.
