苯那普利拉对高血压所致左心室重构及舒张顺应性变化的影响

摘    要:目的:观察苯那普利拉(洛汀新)对高血压所致左心室重构及舒张顺应性变化的影响。方法:50例高血压病伴左心室肥厚的患者,Ⅰ组(治疗组)30例,给予洛汀新10 m g/d,Ⅱ组(对照组)20例,给予硝苯吡啶30 m g/d,治疗6个月后,测定左心室室壁厚度及左心室舒张功能的变化。结果:治疗组血压由2394±479/1425±133 k Pa 降至1835±218/1221±134 k Pa( P< 001),左室后壁厚度由125±25 m m 降至112±23 m m ,室间隔厚度由126±28 m m 降至110±22 m m ( P均< 001),二尖瓣前叶 E F 斜率( E F V)由120±23 m m /s 升至150±40 m m /s, 左室舒张末压( L V E D P)由157±041 k Pa 降至135±020 k Pa( P 均< 001)。而对照组,除血压有显著下降外( P< 001),其他指标均无显著变化( P 均> 005)。结论:洛汀新不但有良好的降压效果,而且可逆转左室肥厚,抑制心肌重构,改善左室舒张顺应性。

Abstract:Objective:To observe the effects of benazepril on diastolic compliance and reconstruction of left ventricle due to hypertension.Methods:Fifty cases were all hypertension coexisting left ventricle hypertrophy that were randomly divided into two groups.Thirty cases in Ⅰ (treated) group were treated with benazepril,10 mg per day;Twenty cases in Ⅱ (control) group,treated by nifedipini,30 mg per day.After 6 months,the thickness of left ventricle wall and the diastolic compliance of left ventricle were determined.Results:The results showed:arterial pressure descended from 23 94±4 79/14 25±1 33 kPa to 18 35±2 18/12 21±1 34 kPa ( P <0 01);the tihckness of left ventricle posterior wall, from 12 5±2 5 mm to 11 2±2 3 mm ( P <0 01);the thickness of ventricular septum from 12 6±2 8 mm down to 11 0±2 2 mm ( P <0 01);early diastolic velocity of mitral valve rose from 120 ±23 mm/s to 150±40 mm/s ( P <0 01);the end diastolic pressure of left ventricle descended from 1.57±0.41 kPa to 1 35±0 20 kPa ( P <0 01) in group Ⅰ,while,there were not significant changes except significantly descended in group Ⅱ.Conclusion:Benazepril can not only descend the arterial pressure,but also reverse the left ventricle hypertrophy,inhibit the reconstruction and improve the diastolic compliance of left ventricle.
