四逆散合半夏泻心汤在呼吸机相关性肺炎治疗中的应用

目的观察四逆散合半夏泻心汤在呼吸机相关性肺炎（VAP）治疗中的作用。方法将60例患者随机分为干预组与对照组各30例。干预组在综合治疗的基础上加用四逆散合半夏泻心汤煎剂，对照组予单纯西医综合治疗。比较两组患者治疗7d的临床肺部感染评分（CPIS）、机械通气时间、达到目标肠内营养量的时间、28d死亡率，并进行统计学分析。结果干预组治疗后CPIS明显低于对照组。机械通气时间、达到目标肠内营养时间短于对照组。28d死亡率低于对照组（均P〈0．05）。结论中药四逆散合半夏泻心汤可改善VAP患者的临床症状．缩短机械通气时间．尽早实现目标肠内营养量．提高免疫功能．从而降低28d死亡率。

Clinical Observation of Sini Powder and Banxiexiexin Decoction Treating Patients with ventilator associated pneumonia

objective： To observe the effects of Sini powder and Banxiaxiexin decoction on patients with ventila- tor associated pneumonia （VAP）. Methods： 60 patients with ventilator associated pneumonia were randomly di- vided into intervention group and control group, 30 cases in each group. Intervention group was given Sini powder and Banxiaxiexin decoction on the basis of comprehensive treatment while control group was only given western medicine comprehensive treatment. Clinical pulmonary infection score（CPIS）,mechanical ventilation time,time to reach the target amount of enteral nutrition,and 28 day mortality rate were compared and analyzed 7 days later. Results： CPIS of intervention group was obviously lower than that of control group after treatment. Mechanical ventilation time and time to reach target amount of enteral nutrition were shorter in intervention group. 28 day mortality rate was lower in intervention group. The difference was statistically significant. Conclusion： Sini pow- der and Banxiaxiexin decoction can improve the clinical symptoms in patients with VAP, shorten the time of me- chanical ventilation, achieve target amount of enteral nutrition as soon as possible,improve immune function, and reduce 28 day mortality rate.
