子宫内膜癌患者外周血CK20 mRNA的表达及临床意义

目的：探讨子宫内膜癌患者外周血CK20 mRNA的表达对子宫内膜癌微转移诊断的价值。方法：采用RT-PCR方法对40例子宫内膜癌患者及20例子宫良性病变患者外周血CK20 mRNA的表达进行检测。并以子宫内膜癌组织和健康女性外周血为对照。结果：子宫良性病变患者及健康女性外周血中均无CK20 mRNA表达；子宫内膜癌组织均有CK20 mRNA表达。40例子宫内膜癌患者术前外周血中，11例检出CK20 mRNA阳性表达，阳性率27．5％。CK20 mRNA阳性检出与临床分期存在显著性相关（P〈0．05），随着临床分期的进展，CK20 mRNA阳性率明显增加。CK20mRNA阳性表达与病理分级无关（P〉0．05）。结论，子宫内膜癌早船可能已出现微量癌细胞的血行播散，CK20可作为子宫内膜癌血行转移的分子标志之一，检测子宫内膜癌患者外周血癌细胞可能成为改进分期、评估预后、提供治疗的参考指标。

Expression of CK20 mRNA in Peripheral Blood of Patients with Endometrial Carcinoma and Its Clinical Significance

Objective:To explore the role of peripheral blood CK20 mRNA for the diagnosis of Endometrial Carcinoma. Methods: RT-PCR was used to detect CK20 mRNA in peripheral blood of 40 patients with endometrial carcinoma as well as 20 patients with benign endometrial disease. Endometrial carcinoma samples and the peripheral blood of healthy women were used as positive and negative controls, respectively. Results: No expression of CK20 mRNA were detected in peripheral blood in healthy women nor in patients with benign endometrial disease. Among the 40 patients with endometrial carcinoma, the positive rates of CK20 mRNA in peripheral blood before radical operation was 27.5% (11/40), respectively. The positive detection of CK20 mRNA was significantly correlated with clinical stage(P<0.05); the more advanced stage, the greater the positive rate. However, there was no significant correlation between the prevalence of CK20 mRNA and pathological level(P>0.05). Conclusion: Blood micro-metastasis of the tumor can present even in an early stage of endometrial carcinoma. CK20 mRNA is a good marker for detecting carcinoma cells in peripheral blood. The detection of CK20 mRNA can be used to guide physician to judge clinical stage, to assess the prognosis and provide effective treatment to patients with endometrial carcinoma.
