显微镜下经鼻蝶入路垂体腺瘤切除术后并发症的影响因素分析

目的探讨显微镜下经鼻蝶入路垂体腺瘤切除术后并发症的影响因素。方法回顾性分析159例行显微镜下经鼻蝶入路垂体腺瘤切除术病人的临床资料．对术后并发症的影响因素进行Logistic回归分析。结果术后共发生并发症44例（27．7％）。经统计学分析：性别、垂体腺瘤卒中、鞍旁侵袭、基础内分泌、手术全切除5种因素与术后并发症的发生相关，其中手术全切除、性别、基础内分泌被纳入回归方程。结论手术全切除、性别、基础内分泌是影响本组经鼻蝶入路垂体腺瘤切除术后并发症的主要因素，其中手术全切除与否对术后并发症的影响较大，应予以重视。

Analysis of factors influencing complications after transsphenoidal surgery under microscope for pituitary adenomas

Objective To investigate the influence factors of complications after transsphenoidal surgery under microscope for pituitary adenomas. Methods Clinical data of 159 patients undergoing transsphenoidal surgery for pituitary adenoma were analyzed retrospectively, and the influential factors for complications were analyzed by Logistic regression method. Results Postoperative complications occurred in 44 patients （27.7%）. Statistical analysis suggested that 5 factors including gender, pituitary adenoma stroke, parasellar invasion, basal endocrine status and total tumor resection were related to postoperative complications, and the factors total tumor resection, gender, and basal endocrine status were included in the regression equation. Conclusions Total tumor resection, gender and basal endocrine status are the main risk factors of postoperative complications after transsphenoidal surgery for pituitary adenomas in this group. Total tumor resection or not is regarded as a greater impact on the postoperative complications, to which we should pay more attention.
