老年腰椎间盘突出症手术治疗

目的探讨老年腰椎间盘突出症（LDH）的手术治疗方法与临床疗效。方法 60例老年LDH患者随机分为观察组与对照组,各30例,观察组行经皮全内窥镜下髓核摘除术治疗,对照组行小切口椎板开窗髓核摘除术治疗,比较两组的疗效。结果两组的近期疗效差异无统计学意义（P〉0.05）;观察组手术切口长度、术中出血量、术后3d腰腿痛VAS评分、术后卧床时间以及住院时间均显著低于对照组（P〈0.05）。结论手术治疗老年LDH疗效显著,相比小切口椎板开窗髓核摘除术,经皮全内窥镜下髓核摘除术创伤小、出血少、术后恢复快,更具应用优势。

Research on surgery for the treatment of elderly patients with lumbar disc herniation

Objective To investigate the surgical treatment and its clinical efficacy for elderly patients with lumbar disc herniation（LDH）. Methods 60 elderly patients with LDH were randomly divided into observation group and control group,30 cases in each group.The observation group underwent percutaneous full-endoscopic discectomy,the underwent small incision intervertebral fenestration discectomy,the efficacy of the two groups were compared the. Results There was no significant difference in short-term efficacy between the two groups（P〉0.05）;the incision length,blood loss,postoperative 3d leg pain VAS score,postoperative bed time and hospital stay of observation group were significantly lower than the control group（P〈0.05）. Conclusion Surgical treatment for elderly patients with LDH has a significant effect,compared to small incision intervertebral fenestration discectomy,percutaneous fullendoscopic discectomy has less invasive,less bleeding,faster recovery,more application advantages.
