地佐辛联合地塞米松用于腹腔镜手术超前镇痛的临床观察

目的观察地佐辛联合地塞米松超前镇痛在腹腔镜手术中应用的临床效果。方法选择腹腔镜手术患者90例,随机分为三组,每组30例。Ⅰ组静脉注射地佐辛0.1mg/kg及地塞米松10mg;Ⅱ组静脉注射地佐辛0.1mg/kg;Ⅲ组为空白对照组。记录三组患者术后2、6、12、24、48h的VAS评分,Ramsay镇静评分,镇痛药物的追加情况及不良反应。结果术后各个时间点,Ⅰ组的VAS评分低于Ⅱ、Ⅲ组（P〈0.05）,镇静效果优于Ⅲ组;Ⅱ组术后2、6h的VAS评分低于Ⅲ组（P〈0.05）;Ⅲ组恶心、呕吐发生率高于Ⅰ、Ⅱ组（P〈0.05）;Ⅰ组镇痛药物的追加低于Ⅱ组和Ⅲ组（P〈0.05）。结论地佐辛联合地塞米松超前镇痛用于腹腔镜手术能有效减轻患者术后疼痛,延长术后镇痛时间,减少镇痛药的使用。

The clinical observation of dezocine combined with dexamethasone in preemptive analgesia of laparoscopic operation

Objective To observate the effect of dezocine combined with dexamethasone on preemptive analgesia in laparoscopic operation in application. Methods 90 cases of patients with laparoscopic operation,were random Ly divided into three groups,30 cases in each group. Group I intravenous dezocine 0.1mg/kg and dexamethasone 10mg; Group II intravenous dezocine 0.1mg/kg; Group Ⅲ was the blank control group. The VAS score,the Ramsay sedation score,additional analgesic drugs and adverse reaction of three groups were recorded in 2,6,12,24,48 h after operation. Results At all time points after operation,the VAS score of group I were lower than those of group Ⅱ and group Ⅲ（P〈0.05）,sedation effect was better than that of group Ⅲ; the VAS score of group Ⅱ in 2,6h after operation were lower than those of group Ⅲ（P〈0.05）; the incidence of postoperative nausea and vomiting of group Ⅲ was higher than that of group I and group Ⅱ（P〈0.05）; additional analgesic drugs of group I were lower than those in group II and group Ⅲ（P〈0.05）. Conclusion Dezocine combines with dexamethasone on preemptive analgesia in laparoscopic operation can effectively reduce postoperative pain,prolong the time of postoperative analgesia,reduce the use of analgesic drugs.
