经皮低温等离子髓核消融术治疗腰椎间盘突出症

目的探讨经皮低温等离子髓核消融术治疗腰椎间盘突出症（LDH）的临床疗效。方法对38例年龄21-68岁的腰椎间盘突出症患者采用经皮等离子髓核消融术治疗,且应用视觉模拟评分法（visual analoguescale VAS）和Macnab评分法对其疗效进行观察分析。结果本组全部患者随访3-18个月,平均11个月。38例患者的症状均不同程度改善。术前VAS评分（6.85±1.25）分,术后随访1-18个月的VAS评分（1.68±0.50）分。根据改良Macnab疗效评定标准,21例治疗效果为优,11例良,4例可,2例差,优良率为84.21%,无一例严重并发症发生。结论在严格掌握适应证的情况下,等离子髓核消融术具有操作简单、安全、创伤小、近期疗效优的特点,是一种有效的治疗腰椎间盘突出症的微创手术。

Percutaneous low-temperature plasma ablation of nucleus pulposus in treatment of lumbar disc herniation

Objective To explore the clinical efficacy of percutaneous low-temperature plasma ablation of nucleus pulposus in treatment of nucleus pulposus intervertebral disc herniation（LDH）. Methods 38 patients（aged from 21 to 68 years old） with lumbar intervertebral disc protrusion received percutaneous plasma ablation of nucleus pulposus,and the clinical efficacy was observed and analyzed by using visual analogue scale（VAS） and Macnab scoring. Results All patients were followed-up for 3-18 months,the average of 11 months. The symptoms of 38 patients had improvement in different degrees. The preoperative VAS score was（6.85±1.25）,all the patients were followed up 1-18 months after operation,the VAS score was（1.68±0.50）. According to the improved Macnab criteria,the treatment effect of 21 cases were excellent,11 cases were good,4 cases were better,2 cases were poor,and the excellent and good rate was 84.21%. There were no severe complications happened. Conclusion Under the condition of strict grasping the indications,Plasma ablation of nucleus pulposus has the characteristics of simple operation,safety,little trauma and superior efficacy,and it is a kind of effective minimally invasive surgery in treatment of lumbar intervertebral disc protrusion.
