被覆食管支架治疗食管良恶性狭窄

目的 探讨被覆食管支架在食管良恶性狭窄中的应用。方法 ２０例食管恶性狭窄和４例良性狭窄患者，于Ｘ线监视下，经口植入被覆食管支架，观察临床效果及并发症。结果 放置支架后患者吞咽困难改善，吞咽程度由术前平均３级降至１级，所有患者皆能进普食或办食。４例有食管－支气管瘘者术后瘘口闭合。术后１例发生支架移位（Ｚ型支架），１例食物嵌塞。随访３～３６个月（平均１４个月），１８例健在，支架均保持通畅。结论 被覆食

Silicone covered stent in the treatment of benign and malignant esophageal strictures

Objective To study the application of silicone covered stents in the treatment of benign and malignant strictures.?Methods In 20 cases of malignant esophageal stricture and 4 cases of benign stenosis, all stents were perorally placed under X ray guidance,and the clinical effects and complications were assessed.?Results After the stent implantation, dysphagia was improved in all of the 24 patients. The mean dysphagia grade fell from 3.0 to 1.0. All patients became able to ingest solid or semi solid foods. The fistula were closed in the 4 patients with esophago-respiretory fistula. Serious complications occurred in 2/24 patients including stent migration and food impaction. The patients were followed up for 14 months (range 3-36 months) and 18 of them were still living with the stents all in good condition.?Conclusion The covered metallic stents are easy to insert,safe and effective for palliative treatment of esophageal strictures and esophago-respiratory fistula.
