TPS与CYFRA21-1对肺鳞癌临床诊断价值研究

目的:探讨组织多肽特异性抗原(tissue polypeptide specific antigen,TPS)、细胞角蛋白19(cytokeratin19 fragment,CYFRA21-1)和鳞状细胞癌抗原(SCC-Ag)在肺癌患者血清中的表达水平及其对肺癌诊断的临床应用价值。方法:采用酶联免疫吸附法(ELISA)和免疫放射分析法(IRMA)对130例肺癌患者、67例肺部良性疾病患者及49例正常人的血清TPS,CYFRA21-1和SCC-Ag浓度进行检测。结果:肺癌组血清TPS和CYFRA21-1水平均明显高于肺部良性疾病组(P<0.01)和正常对照组(P<0.01)。TPS,CYFRA21-1和SCC-Ag阳性率与浓度均随着临床分期的进展而升高。TPS和CYFRA21-1在化疗前后和手术前后有明显变化。结论:肺癌患者血清TPS和CYFRA21-1阳性检出率较高,是诊断肺癌的一种敏感性强的肿瘤标志物。TPS和CYFRA21-1联合检测肺癌有重要意义。

The Value of TPS and CYFRA21-1 to Lung Squamous Carcinoma in Clinical Diagnosis

Objective: To investigate the expression of tissue polypeptide specific antigen(TPS),cytokeratin 19 fragment(CYFRA21-1)and squamous cell carcinoma antigen(SCC-Ag) in patients with lung cancer and the clinical value to the diagnosis of lung cancer.Methods: The levels and positive rate of serum TPS,CYFRA21-1 and SCC-Ag were determined by enzyme-linked immunosorbent assay(ELISA) and immunoradiometric assay(IRMA) in 130 patients with lung cancer,67 patients with benign diseases and 49 healthy adults. Results: The levels of serum TPS and CYFRA21-1 in lung cancer group were significantly higher than those in the normal control group(P<0. 01) and the benign lesion group(P<0.01).And the levels and positive rate of TPS,CYFRA21-1 and SCC-Ag became even higher as the malignancy progressed.Before and after the chemical therapy and operation,the levels of TPS and CYFRA21-1 changed greatly.Conclusions: The positive detectable rate of serum TPS and CYFRA21-1 is very high in patients with lung cancer and TPS and CYFRA21-1 can be used as a very useful and sensitive tumor marker in diagnosis of lung cancer.The combining test of TPS and CYFRA21-1 has an important role in diagnosis of lung cancer.
