耐亚胺培南鲍曼不动杆菌同源性及耐药机制研究

摘    要:目的调查耐亚胺培南鲍曼不动杆菌在各科室的流行特点,研究其耐药机制。方法收集不同科室分离的耐药菌54株,采用Vitek-2Compact进行药敏实验;采用PFGE分析同源性;PCR及DNA测序检测耐药基因型。结果PFGEA型占77.8%,B型占18.9%。54株菌都携带OXA-66基因,53株菌携带OXA-23基因,41株菌携带16SrRNA甲基化酶annA基因。结论临床科室存在A型耐药菌的克隆播散。产OXA-23型碳青霉烯酶基因及其上游插入序列ISAbaI是其最主要的耐药机制。

Study on the resisitance mechanism and homology of imipenem-resistant acinetobacter baumannii

Abstract:Objective To investigate the resistant mechanism to carbapenems and molecular epidemiological characteristics of imipenem-resistant acinetobacter baumannii from clinical isolates.Methods A total of 54 imipenem-resistant acinetobacter baumannii isolates were collected from different clinical departments.The Vitek-2 Compact identify bacteria,susceptibility testing and determination of MIC values;Pulsed-field gel electrophoresis was performed to analyze homology of these isolates;PCR and DNA sequencing were used to detected several carbapenemase genes and 16S rRNA methylase genes.Results Genotype A distributed in various departments as a majorwas epidemic clone.All isolates contained the blaoxa-66 carbapenemase gene,53 isolates contained blaoxa-23 and ISAbaⅠuspstream of the gene,16S rRNA methylase gene armA was identified in 41 IRABs isolates.Conclusion There has been clonal A spread of IRAB strain among patients in the hospital.Most of imipenem-resistant acinetobacter baumannii contained blaoxa-23 and ISAbaⅠuspstream of the gene.
