后腹腔镜下肾囊肿去顶术29例报告

目的探讨经后腹腔镜下肾囊肿去顶术的方法，提高手术技巧。方法29例肾囊肿患者于全麻下行经后腹腔镜下肾囊肿去顶术。男性12例，女性17例。年龄42～76岁，平均61岁。左侧16例，右侧13例，囊肿大小5．0cm×5．2cm～7．8cm×9．1cm。结果29倒手术均成功。手术时间30—75min，平均42min。术中出血平均30ml。术后住院时间3～10d，平均5d。无严重手术并发症。29例随访5～36个月无复发。结论经后腹膛途径行腹腔镜肾囊肿去顶术具有创伤小、出血少、术后恢复快、疗效确切等优点，是治疗肾囊肿的理想方法。

Laparoscopy by retroperitoneal access for treatment of renal cyst: Report of 29 cases

Objective To discuss the techniques of retroperitoneal laparoscopic operation in treatment of renal cyst and to improve the skill of endoscopic surgery, Methods 29 cases of renal cysts （ 12 men and 17 women, age range from 42 - 76 years with mean age of 61 years）underwent retroperitoneal laparoscopie unroofing and decompression. The size of cysts ranged from 5. 0 cm × 5.2 cm to 7.8cm × 9. 1 cm. Cysts were located on left side in 16 cases and on right side in 13 cases. Results The successful rate of operation was 100% . The operation time was 30 - 75 min （ with a mean of 42 min ）. The mean blood loss during the operation was 30 ml. The postoperative hospitalization was 3 - 10 d days （ with a mean of 5 days）, All 29 patients were followed up for 5 - 36 months, and no recurrence occurred. Conclusion The retroperitoneal laparoscopic unroofing of renal cysts has advantages of minimal trauma, less suffering, less blood loss and rapid recovery, so it is the ideal choice for treatment of renal cvsts.
