抗双链DNA抗体不同检测方法的对比分析及临床应用研究

目的探讨胶体金斑点法(DIGFA)和酶联免疫吸附法(ELISA)检测抗双链DNA(dsDNA)抗体的临床应用价值。方法采用DIGFA法和ELISA法同时检测系统性红斑狼疮(SLE)患者(n=80)及健康对照者(n=50)血清中抗dsDNA抗体。并将ELISA法的定量检测结果与SLE患者的临床指标进行相关分析。结果 ELISA法检测抗dsDNA抗体的特异性、阳性预测值均高于DIGFA法,但敏感性略低于后者。ELISA法检测的抗dsDNA抗体水平与SLE患者疾病活动性指数、血沉、血清C3及24小时尿蛋白定量结果呈明显相关性。结论 ELISA法检测抗dsDNA抗体水平较DIGFA法准确性更高,并且是监测SLE病情转归的重要实验室指标之一。

Comparasion of detecting methods for anti-double stranded DNA antibody and clinical application

Objective To explore the application value of anti-do uble stranded DNA（dsDNA） antibody detection by dot immunogold filtration assay （DIGFA） and enzyme linked imunosorbent assay（ELISA）.Methods D IGFA and ELISA were employed to detect anti-dsDNA in serum sample of systemic l upus erythematosus（SLE） group（n=80） and healthy control group（n=50）.S pearman’s correlation was used for exploring the relationship between the ELISA results and disease activity of the SLE patients.Results The E LISA results showed higher specificity,positive predictive value and lower sent ivity than that of DIGFA.The anti-dsDNA level of SLE patients detected by ELIS A showed significant correlation with the level of SLE disease activity index（S LEDAI）,ESR,Serum complement C3 and 24-hr urine protein of the SLE patients.Conclusion ELISA is more accurate than DIGFA in detection of anti-dsDNA.It can be regarded as one of the monitor of SLE disease activity.
