2008年洛川县大骨节病病情调查分析

目的进一步掌握洛川县病情消长态势和环境因素对大骨节病发生、发展的影响,为大骨节病的防治提供科学依据。方法参照洛川县大骨节病病区资料,确定调查点进行临床普查,并采集粮食样本及儿童头发样本进行检测,测定粮硒、发硒及相关因素与病情的关系。结果临床检查≥16岁以上人群293人,其中大骨节病临床Ⅰ度以上患者21人,Ⅱ度患者7人,Ⅲ度患者1人,总检出率为9.89%。儿童发硒为0.34±0.11μg/g,粮食0.038±0.016μg/g。旧县镇院夫村大骨节患病以36～60岁人群较高,20岁以下没有新发病例。结论儿童大骨节病病情处于基本控制后的平稳趋势。病区外环境硒含量水平的明显升高、饮食结构的明显改善、粮食卫生学质量的提高及经济收入的增加是病情得到控制的主要因素。

The Investigation on the State of Kaschin-Beck＇s Disease in 2008 of Luochuan County

Objective For providing a scientific basis on the prevention and control of the Kaschin-Beck＇s disease and understanding the trend and the impacts of the environment facts on Kaschin-Beck＇s disease onset and development in Luochuan county. Method Referred historical Kaschin-Beck＇s disease data of the endemic area in Luochuan county,the investigate village was selected to carry out the clinical census. Detected the sample of food and children＇s hair of the investigate point. Results 293 residents older than 16 were taken to clinical examination. Among these residents 21 patients were of Ⅰ stage Kaschin-Beck＇s disease,7 patients were of Ⅱ stage Kaschin-Beck＇s disease,and 1 patient was of Ⅲ stage Kaschin-Beck＇s disease,and accounted for the 9.89% in total. The morbidity rate of Kaschin-Beck＇s was higher in 36-60 years old group of yuanfu village in jiuxian town,and there was no new case in the residents who was younger than 20 years old. Conclusions The pathogenetic condition of Kaschin-Beck＇s disease in children was steady after the basic control. The obviously raised of selenium contents of the external environment,improvement of the dietary pattern ,enhanced of the food sanitation and increased of the financial reward were all the major factors for the disease controlled.
