保留肋间臂神经在乳腺癌根治术中的临床应用

目的探讨在乳腺癌根治术中行肋间臂神经保留的临床疗效及其临床意义。方法分析本科2011年1月至2012年12月行乳腺癌根治术的60例患者的临床资料,将其分为观察组与对照组。观察组30例,均在术中行肋间臂神经保留;对照组30例,均在术中切除肋间臂神经,对2组进行观察及后期随访。结果观察组与对照组平均手术时间、平均淋巴结清扫数无明显差异;术后患侧肢体感觉异常情况、疼痛情况,观察组明显低于对照组,差异有统计学意义(P0.05);术后随访1~3年,部分病例出现局部复发及远处转移,但2组比较无明显差异。结论乳腺癌根治术中肋间臂神经的保留,能够明显降低患侧肢体的感觉障碍,提高术后生存质量,且不明显延长手术时间,也不增加肿瘤的复发及转移率,值得广泛推广。

Clinical value of intercostobrachial nerve preservation in breast cancer operation

Objective To investigate the clinical significance of reserving intercostobrachial nerve（ ICBN） during breast cancer operation. Methods A total of 60 cases of breast cancer from January 2011 to December 2012 in our hospital were selected,and they were divided into the control group and the observation group with 30 cases in each group. The control group didn＇t preserve the intercostobrachial nerve. The observation group preserved the intercostobrachial nerve.Results The mean operative time and mean number of dissected lymph node between the observation group and the control group had no significant difference. The incidence rate of postoperative ipsilateral limb paresthesia and pain in the observation group was significantly lower than that in the control group after 3,6 and 12 months（ P〈0. 05）. Conclusions Breast cancer operation preserving ICBN can significantly reduce the limb sensory impairments,and improve the quality of life of the patients,and does not prolong the operation time or increase the rate of tumor recurrence and metastasis,which is worthy of clinical application.
