91例重度窒息新生儿凝血功能及生化指标分析

摘    要:目的: 分析91例重度窒息新生儿凝血功能及生化指标。方法: ①对91例重度窒息新生儿凝血功能及生化指标进行回顾性分析, 并与经典参考值比较。结果: ①91例重度窒息新生儿均有不同程度并发2 ~8种各系统器官损害并发症; ②91例重度窒息新生儿凝血功能(PT、TTT、APTT、FIB) 与经典参考值比较, 均有着非常显著性差异(P<0 .01); ③重度窒息并发肾衰患儿肾功能(BUN、Cr、UA) 与经典参考值比较, 都有着非常显著性差异(P<0 .01); ④重度窒息并发各系统器官损害的生化指标(AST、CK、LDH、ALT、高GLU) 与经典参考值比较, 均有着非常显著性差异(P<0 .01), 低GLU、低K+、高K+、低Ca++与经典参考值比较均有显著性差异(P<0 .05)。结论: 窒息后新生儿易并发各系统器官损害并发症。凝血功能应作为首选监测指标, 对防止新生儿在重度窒息及窒息后各系统器官损害, 有着指导性的重要临床意义。

Abstract:Objective:To analyze the test results of blood coagulation function and blood biochemistry analysis of 91 cases newborn babies.Methods:The retrospect analysis was used for clinic study of the above cases.Results:①The function damages of 2-8 years of organs were involved in 91 cases newborn with severe asphyxia, ② the abnormality of their coagulation function occured (compared with the normal newborn babies), ③to those newborn babies with severe asphyxia accompanied by dysfunction of kidney, the serum levels of BUN (11.39±3.66mmol/L), Cr(152.93±45.38 mmol/L)and UA(505.0±231.2 mmol/L) showed very high abnormality compared with the normal level( P <0.01). Furthermore, the results of biochemical test among the disorder newborn babies with the systemic damage of organs and tissues had much more variant than that of health newborn babies ( P <0.01), for example, AST(163.26±156.8 U/L), CK(2578.52±5246.85 U/L), LDH(860.66 4±846.22 U/L), ALT(71.64±23.28 U/L), GLU (10.63±3.96 mmol/L). Sometimes the results showed very large difference( P <0.05),for example, low GLU (2.20±0.69 mmol/L), low K +(2.29±0.31 mmol/L),low Ca ++ (1.48±0.19 mmol/L) and high K +(6.13±0.4 mmol/L).Conclusion:The newborn babies with heavy asphyxia are very easy to suffer from various syndromes from different kind of organs or tissues. So that, detecting the function of blood coagulation as the first monitor indexes are very necessary for disorder newborn babies.
