原发性高血压患者颈动脉粥样硬化与血清铁蛋白和同型半胱氨酸的相关性研究

摘    要:目的通过观察原发性高血压(EH)患者颈动脉内膜中层厚度(CAIMT)和血清铁蛋白(SF)、同型半胱氨酸(Hcy)的水平,探讨EH患者颈动脉粥样硬化(CAS)与血清SF和Hcy的相关性。方法以266例EH患者为研究对象,检测CAIMT(彩色多普勒超声仪)和血清SF(电化学发光免疫法)、Hcy(循环酶法)水平,据CAIMT结果分组后进行统计学对比分析。结果 1对照组、CAIMT非增厚组及CAIMT增厚组血清SF和Hcy水平依次升高,差异有统计学意义(F=69.171、38.618,P〈0.01),CAIMT增厚组显著高于CAIMT非增厚组(t=12.561、4.581,P〈0.01);CAIMT非增厚组血清SF和Hcy水平均分别显著高于对照组(t=6.305、2.913,P〈0.01)。2CAIMT分别与血清SF和Hcy呈正相关(r=0.597、0.451,t=9.560、4.533,P〈0.01)。结论 EH患者体内存在着高水平的SF和Hcy,CAS的发生与血清SF和Hcy水平显著相关,血清SF和Hcy联合检测有助于对病情和预后的判断,改善其水平可能是防治或延缓EH合并CAS的策略之一。

Abstract:Objective To investigate the relationship between carotid atherosclerosis(CAS) and serum SF, Hcy in patients with essential hypertension (EH) by observing the carotid artery intima-media thickness ( CAIMT), serum SF and Hcy levels. Methods Two hundred and sixty-six EH patients were chose as the research object, the CAIMT( color doppler ultrasound instru- ment) and serum SF( electrochemiluminescence immunoassay) and Hcy( circulating enzymatic) levels were detected, and statis- tical comparison analysis was made according to the results of CAIMT grouping. Results SF and Hcy levels increased succes- sively in control group, CAIMT non-thickening group and CAIMT thickening group, the differences were significant ( F = 69. 171,38. 618, P all 〈 0. 01 ), and it increased obviously in CAIMT thickening group CAIMT than that in the CAIMT non- thickening group ( t = 12. 561,4. 581, P all 〈0. 01 ). The serum SF and Hcy levels in CAIMT non-thickening group were signifi- cantly higher than those in control group( t = 6. 305,2. 913, P all 〈 0. 01 ). CAIMT were positively correlated with serum SF and Hcy ( r = 0. 597,0.451, t = 9. 560,4. 533, P 〈 0. 01 ). Conclusion The levels of serum SF and Hcy in EH patients are high, the occurrence of CAS is significantly related to the levels of serum SF and Hcy. Combined detection of serum SF and Hcy levels is helpful to judge the condition and prognosis, so improving the levels of SF and Hcy may be one of the methods to prevent or delay the development of EH complicated with CAS.
