卡维地洛和美托洛尔治疗高血压的疗效及其对糖脂代谢影响的随机对照研究

目的探讨卡维地洛和美托洛尔治疗高血压的疗效及其对糖脂代谢的影响。方法将120例高血压患者按照随机数字表法随机分为对照组和治疗组,每组60例。治疗组给予卡维地洛治疗,对照组给予美托洛尔治疗,比较两组患者血压控制情况及血糖、血脂水平。结果治疗组治疗有效率为91.67%,对照组为78.33%,差异有统计学意义（P〈0.05）。两组患者治疗前血压比较差异无统计学意义（P〉0.05）;但治疗后两组血压均下降,且治疗组下降较对照组明显,差异有统计学意义（P〈0.05）。两组患者治疗前血糖、胰岛素（INS）、胰岛素敏感指数（ISI）、总胆固醇（TC）、三酰甘油（TG）、低密度脂蛋白胆固醇（LDL-C）、高密度脂蛋白胆固醇（HDL-C）指标比较差异无统计学意义（P〉0.05）,但治疗后各项指标比较差异有统计学意义（P〈0.05）。对照组患者治疗后血糖、TC较治疗前升高,且差异有统计学意义（P〈0.05）。治疗组患者治疗后INS、TC、TG、LDL-C较治疗前大幅度降低,而ISI、HDL-C却大幅度升高,差异有统计学意义（P〈0.05）。结论卡维地洛降压效果更佳,且对糖脂代谢影响小,可作为高血压治疗的一线药物。

Effectiveness of carvedilol and metoprolol on hypertension and glucolipid metabolism:a randomized controlled study

Objective To investigate the efficacy of carvedilol and metoprolol on hypertension and glucose and lipid metabolism. Methods One hundred and twenty patients with hypertension were randomly divided into control group and treatment group, with 60 cases in each group. The patiems in treatment group were treated with carvedilol, and the patients in control group were treated with metoprolol. The blood pressure control and blood glucose and lipid levels were compared between the two groups. Results The effec- tive rate in treatment group was 91.67%, and the effective rate in control group was 78.33%, the difference between the two groups was statistically significant（P 〈 0. 05 ）. There was no significant difference in blood pressure between the two groups before treatment （P 〉 0. 05）, but the blood pressure decreased after treatment, and the blood pressure in the treatment group was significantly lower than that in the control group, the difference was statistically significant （P 〈 0. 05 ）. There was no significant difference in Glu, INS, ISI, TC, TG, LDL-C, HDL-C index before treatment （P 〉0. 05）, but there were significant differences after treatment （P 〈0. 05）. Glu, TC in the control group increased than that before treatment, and the difference was statistically significant （P 〈 0.05）. INS, TC, TG, LDL-C significantly reduced, while the ISI, HDL-C levels were significantly increased after treatment, the differences were statis- tically significant（P 〈0.05）. Conclusion The antihypertensive effect of carvedilol is good, and has little effect on glucose and lipid metabolism, can be used as first-line treatment of hypertension drugs.
