幽门螺杆菌细胞毒素相关基因、空泡毒素基因及其表达产物与儿童胃十二指肠疾病的关系

摘    要:目的 研究幽门螺杆菌 (Hp)的细胞毒素相关基因 (cagA基因 )、空泡毒素基因 (vacA基因 )及其表达产物cagA蛋白、vacA抗体与儿童消化性溃疡 (PU)、慢性胃炎 (CG)之间的关系。 方法 对 15 2例Hp感染患儿 (PU 31例 ,CG 12 1例 )的胃黏膜应用 4对不同引物 ,通过聚合酶链反应 (PCR)技术检测cagA、vacA基因 ;用免疫印迹法测定 139例血清cagA、vacA抗体。结果  15 2例胃黏膜 10 0 %具有vacA基因 ( 15 2 /15 2 ) ,cagA基因经用 3对不同引物检测 ,其总阳性率为 78% ( 118/15 2 ) ,其中PU 71% ( 2 2 /31) ,CG 79% ( 96 /12 1) ,两组间差异无显著性 ( χ2 =0 .996 P >0 .0 5 )。 139例血清vacA抗体阳性率 92 .1% ( 12 8/139) ,PU 86 .7% ( 2 6 /30 ) ,CG 94 .5% ( 10 3/10 9) ( χ2 =1.14 6 P >0 .5 ) ;cagA抗体阳性率 91.4 % ( 12 7/139) ,PU 83.3% ( 2 5 /30 ) ,CG93.6 % ( 10 2 /10 9) ( χ2 =3.130 P >0 .5 ) ,以上两组间差异均无显著性。结论  1.浙江地区儿童感染的Hp绝大多数为具有cagA基因的毒力菌株。 2 .亚洲地区与西方人群中感染的Hp菌株可能存在等位基因的多态性、变异性

Cytotoxin associated protein gene,vacuolating cytoxin gene and expression production of helicobacter pylori correlative with gastroduodenal disease in children
Abstract:Objective To study the relationship between expression products of Cytotoxin associated protein(cagA) gene,Vacuolating cytoxin(vacA)gene and peptic ulcer disease(PU),chronic gastritis(CG).Methods Gastric biopsy specimens were obtained from 152 children with helicobacter pylori(Hp) infection, including 31 with PU,121 with CG.The gene of cagA and vacA were examined by polymerase chain reaction(PCR) using 4 different primer sets in 152 specimens.cagA and vacA antibody was detected by western blot analysis in 139 serum of 152 children.Results Among 152 children, the positive rate of vacA gene was100 % The positive rate of cagA gene with 3 pairs of primers was 78 % (118/152).There was no significant difference between PU(71 %,22/31) and CG( 79 % ,96/121)( P >0.05). Among 139 serum, the positive rate of vacA antibody was 92.1 %(128/139)( P >0.05),There was no significant difference between PU(86.7 %,26/30) and CG(94.5%,103/109). The positive rate of cagA antibody was 91.4 %(127/139);There was also no significant difference between PU(83.3 %,25/30) and CG(93.6 %,102/109),( P >0.5).Conclusions 1. Most of the Hp infected children carried cagA gene in Zhejiang region. 2.Compared with CG, the positive rates of the gene and the protein of cagA and vacA in PU are not significantly different.
