TVI观察环磷腺苷葡甲胺对二尖瓣环运动的影响

摘    要:目的应用TVI技术测量二尖瓣环6个节段的运动速度来观察静脉应用环磷腺苷葡甲胺对心脏功能的影响。方法应用TVI技术测量30例心脏病患者用药前后二尖瓣环后间隔、前间隔、前壁、侧壁、后壁、下壁各位点的Sa、Ea、Aa及Ea/Aa之比。结果(1)用药后二尖瓣环6个节段的平均Sa均有显著性增加(P<0.01)。(2)Ea、Aa参数虽然也有明显增加,但Ea/Aa之比无显著性差异(P>0.05)。(3)Sa与左室射血分数呈良好相关性(r=0.76),Ea/Aa之比与二尖瓣血流流速曲线E/A之比亦呈良好相关(r=0.85)。结论环磷腺苷葡甲胺能明显改善左室收缩功能。应用TVI技术测量二尖瓣环6个节段的平均运动速度可定量评价左室整体收缩和舒张功能。该方法与传统超声心动图测量左室功能有良好相关性。

Abstract:Objective Tissue Velocity Imaging(TVI)is applied to assess the motion of 6 different sites of m itral annulus to evaluate the cardiac function by intravenous meglumine cyclic adenylate(MCA). Methods TVI and 2-DE were performed in 30 patients with heart disease before and after intravenous MCA infusion. Sa, Ea, Aa and Ea/Aa ratio of 6 mitral annulus sites (posterior septal, anterior wall, anteroseptum, lateral wall, inferior wall and posterior wall) were measured respectively. Results 1. Average Sa of each of 6 mitral annulus sites significantly increased after MCA infusion (P<0.001) 2. The parameter of Ea, Aa increased, but no difference was found in Ea/Aa ratio (P>0.05) 3. There were close relations between the average Sa and LVET (r=0.76,P<0.01) and between the average Ea/Aa of mitral annulus and transmitral index E/A ratio (r--0.85,P<0.01) Conclusions The left ventricular systolic function is improved by MCA. The 6-site average velocities of mitral annulus measured by TVI can quantitatively assess the global systolic and diastolic function of left ventricle and has a good correlation between the TVI and conventional echocardiography.
