彩色多普勒在胆管细胞癌诊断与鉴别诊断中的价值

目的　探讨彩色多普勒超声在胆管细胞癌诊断与鉴别诊断中的意义。方法　对 3 3例胆管细胞癌与 18例肝内胆管结石患者的彩色多普勒血流进行对比观察。结果　胆管细胞癌组彩色多普勒显示病变内门静脉血流显像不清的为 90 .91% (3 0 / 3 3 ) ,其中 76.67% (2 3 / 3 0 )肝动脉亦显像不清 ,相反 2 3 .3 3 % (7/ 3 0 )显示丰富的肝动脉血流 ;然而肝内胆管结石组中门静脉、肝动脉均显像不清的仅为 11.11% (2 /18) ,2组比较差异有非常显著性意义 (P<0 .0 0 1)。结论　门静脉血流信号变细或消失为胆管细胞癌的彩色多普勒特征性表现 ,其在胆管细胞癌的诊断与鉴别诊断上具有重要的临床价值

Value of Color Doppler in Diagnosis and Differential Diagnosis of Cholangiocarcinoma

Objective:To probe the value of color Doppler in diagnosis and differential diagnosis of cholangiocarcinoma.Methods:The color Doppler ultrasonographic features of 33 cases with cholangiocarcinoma and 18 cases with cholelithiasis were compared.Results:The color Doppler blood flow of portal veins was not dis played clearly in 90 91%(30/33)of cholangiocarcinoma in which the color Doppler blood flow of the hepatic arteries in 76 67%(23/30)of the cases was also not d isplayed clearly.Yet the color Doppler blood flow of the portal veins and hepati c arteries in only 11 11%(2/18) of cholelithiasis was not displayed clearly.The re was a markedly difference between the two groups(P<0 01).Conclusions:It is the characteristic property that color Doppler blood flow of protal vein becomes slender or disappeared in the cholangiocarcin oma.Color Doppler ultrasound possesses important significance in diagnosis a nd differential diagnosis of cholangiocarcinoma.
