胃粘膜隆起病变分型鉴别意义

目的探讨胃部粘膜非蒂状隆起病变的分型、鉴别诊断、预后及其临床意义。方法对１６２０人次内镜检查所发现的１３２个胃粘膜隆起病变均作病理检查，分为息肉样（ｎ＝９２）、疣状（ｎ＝２５）和扁平状（ｎ＝１５）３型，通过治疗、定期检查，对各型隆起病变预后进行对照分析。结果各型隆起病变的转归与病变性质密切相关，疣状隆起病变的肠腺化生、非典型增生等癌前变化明显高于息肉样和扁平状隆起病变（Ｐ＜００１），癌前变化与胃粘膜屏障的完整性和血循环、细胞修复功能、Ｈｐ感染等内环境有关，具有一定可逆性。结论对胃粘膜隆起病变的正确诊断、分型并给予早期治疗、定期复查具有重要的临床意义

The differential diagnostic significance of stomach convex lesions typing

AIMS The different types of none stalk stomach convex lesions and its different diagnosis, results as well as clinic significance were observed. METHODS One hundred and thirty two none stalk stomach convex lesions found out from 1620 patients' endoscopic examinations were firmed pathologically and divided into three types: polyform(n=92), verruciform (n=25) and flateform (n=15). Through treatment and follow_up endoscopic examinations were analyzed. RESULTS The prognosis of stomach convex lesions each type's was closely correlated to its pathologic characters and the intestinal metaplasia as well as atypical hyperplasia in verruciform was evidently more serious than those in other two (P<0 01). These changes were correlated with the factors such as complete gaster membrance, blood circulation and epithelio_glandular renovate function CONCLUSIONS The important clinicsal significance of none stalk stomach convex lesions lies in correct diagnosis, early treatment and regular intervallic endoscopic examination.
