应用红外热像技术评估糖尿病合并下肢血管病变患者治疗效果与预后

摘    要:背景目前对于糖尿病肢体血管病变,临床上尚缺乏可靠、迅速的评价方法. 目的探讨红外热像技术在糖尿病肢体血管病变诊断及预后评价方面的应用. 设计以诊断为依据的病例对照研究. 地点、对象和方法选择在解放军第三军医大学附属新桥医院内分泌科住院治疗过的合并轻度血管病变的糖尿病患者 10例,合并重度血管病变的糖尿病患者 10例,糖尿病多并发症患者 20例,无并发症糖尿病患者 20例进行双下肢红外线热像检查.而后对有并发症患者予以并发症治疗,作为治疗组;对无并发症患者予以降糖治疗,作为对照组,治疗结果用红外热像仪进行评价,观察治疗前后两组间的肢体温差变化. 主要观察指标两组患者治疗前后红外热像图观察的肢体变化. 结果治疗前两组患者热像检查结果比较,差异无显著性意义( P >0.05).治疗后治疗组与对照组的肢体温差 (0.27± 0.26) ℃ ]较治疗前明显减小 (t=6.13,P< 0.001).可见微循环明显改善. 结论红外热像技术对于糖尿病肢体血管病变的诊断及效果评价是一种可靠、迅速的方法.

Abstract:BACKGROUND:There is no clinical reliable and rapid evaluating method available in diabetes limbal vascular pathological changes recently. OBJECTIVE:To discuss the applications of thermal infrared image technique in the diagnosis of diabetes limbal vascular pathological changes and the evaluation of prognosis. DESIGN:A case-controlled study on the basis of diagnosis. SETTING,PARTICIPANTS AND METHODS:Ten cases of diabetes complicated with minor vascular pathological changes,10 cases of diabetes complicated with severe vascular pathological changes and 20 cases of diabetes complicated with multiple complications as well as 20 cases of diabetes without complications were selected from endocrinology inpatient department of Xinqiao Affiliated Hospital of the Third Military Medical University to be examined with thermal infrared imagery on two lower limbs.Patients with complications were treated by complication therapy and allocated into therapy group;patients without complications were treated by routine diabetes therapy and allocated into control group.The therapeutic results were evaluated by thermal infrared imager to observe the body temperature changes between two groups after therapy. MAIN OUTCOME MEASURES:The changes of thermal infrared images of two groups of patients before and after therapy. RESULTS:There was no significance between the thermal infrared images between two groups before treatment(P >0.05).The differences in body temperature in the two groups after treatment(0.27± 0.26) ℃ ]significantly decreased compared with those before treatment(t=6.13,P< 0.001),which indicated a significant improvement of microcirculation. CONCLUSION:Thermal infrared image technique is a reliable and rapid method in evaluation of diagnosis and therapeutic effectiveness of vascular pathological changes in diabetes patients.
