Wegener肉芽肿的肺部CT表现

目的:回顾性分析Wegener肉芽肿肺部CT征象,并结合文献探讨CT诊断的可能性。方法:8例经皮穿刺活检证实的Wegener肉芽肿于穿刺前进行了CT平扫检查,其中6例还进行了增强扫描。结果:7例表现为肺内多发性病灶,其中6例为结节性,1例为斑片状,余1例为肺孤立性灶。5例病灶内可见空洞,1例伴有楔形阴影,1例显示“供养血管征”。增强扫描多数结节呈边缘性强化。结论:多发性肺结节、结节性内空洞,伴有楔形阴影或浸润性病变以及病变的演化规律和强化特征为Wegener肉芽肿的特点。CT具有重要的诊断意义。

The lung CT manifestation of Wegener's granulomatosis

Objective:To retrospectively review the lung CT manifestation of Wegener's granulomatosis and to evaluate the probability of the diagnosis by CT.Methods:The lung CT manifestations of 8 patients with Wegener's granulomatosis verified by pathology were retrospectively analyzed.Results:Multiple pulmonary lesions of soft dense appeared in 7 cases on CT scans,one case has single lesion.Cavities were commonly seen in the larger nodules with 5 patients,the peripheral wedge-shaped shadows in the 1 case of them,and single lesion presented with "vascular sign".The most nodules demonstrated central hypodensity and perpheral enhancement.Conclusion:Multiple pulmonary nodules,some of which present cavities,accompanied by wedge-shaped shadows or massive shadow with blurred margins,as well as the law of change and the features of enhancement are relatively characteristic to Wegener's granulomatosis.CT can be provided with significant function for diagnosis of Wegener's granulomatosis.
