尿可溶性抗原检测早期诊断军团菌肺炎的实验研究

为探讨应用生物素-亲和素的酶联免疫测定法检测军团菌可溶性抗原对早期诊断军团菌肺炎价值，用BA—ELISA法检测27只Lp1型军团菌肺炎豚鼠尿标本，发现感染后第1d36．8％（7／19）的豚鼠尿中可测出军团菌可溶性抗原，至感染后第5d达83.3％（20/24），累计敏感性达100％。47只非军团菌感染豚鼠尿检示该法的特异性为91.5％（43／47），与金黄色葡萄球菌（金葡菌）及某些假单胞菌有交叉反应，表明BA—ELISA法检测尿军团菌抗原具有较高和较早的诊断价值。

DETECTION OF URINARY ANTIGEN OF LEGIONELLA FOR EARLY DIAGNOSIS OF LEGIONELLA PNEUMONIA

PURPOSE To study the value of detecting the soluble antigen of legionella pneumophila in urine using BAELISA for early diagnosis of infection with legionella pneumonia.METHODS With BA- ELISA method, the urine samples of 27 guinea pigs in which Lp1 were positive inculture were detectedRESULTS The soluble antigens could be detected in the first day after inoculation of the guinea pigs. Thepositive rate of urine samples was 36. 8 %. It approached to 83. 3 % in the sib day after inoculation and the accumulated sensitivity rate reached 100 % during this period. The detection of urine samples of 47 guinea pigswith non - legionella pneumonia showed that the specificity of the BA - ELISA for the urinary Lp1 antigen was91. 5 % (43/47) , with cross - reaction with other respiratory pathogens i. e. St. aureus, P. aeruginosa.CONCLUSIONS The study indicates the detection of urinary soluble antigen of Lp1 by BA - ELISA appears to be a better method for the early dioposis of legionella pneumonia with high sensitivity and specificity.
