选择性脊神经根切断术治疗肢体痉挛

目的：利用臂丛神经根相互代偿的原理设计了选择性臂丛神经根切断术以及腰骶神经根选择性切断术治疗肢体痉挛的手术方法。方法：根据患者临床表现，设计了五种手术方法，１例选择Ｃ７神经根切断术，４例选择Ｃ８神经根单根切断，４例选择Ｃ８神经根分束切断，２例选择Ｃ６、Ｃ８神经根同时切断，２例下肢痉挛的病例采用一侧选择背根切断、一侧选择腰骶神经根切断术，对１３例患者进行治疗。结果：术后经２个月～１年的随访发现，所有患者肢体痉挛均明显减轻，部分患者功能得到明显改善。结论：臂丛神经根切断尤其是Ｃ８神经根分束切断治疗手局部痉挛可使手局部痉挛消失或减轻，功能得到部分恢复，是较为理想的治疗手局部痉挛的方法。

The Treatment of Limb Spasticity by Selective Nerve Roots Rhizotomy

Objective: To confirm the efficacy of selective brachial plexus and lumber nerve root rhizotomies in reducing spasticity of upper and lower extremities in thirteen cases. Methods: Five kinds of operation had been designed according to patients' symptoms: C 7 rhizotomy was performed in 1 case , C 8 nerve root rhizotomy in 4, C 8 fascicle partial section in 4, C 6 and C 8 rhizotomy in 2, nerve root rhizotomy of L 5 and S 1 was done for 2 cases of spasticity of the lower limbs. Results: At 2 months- 1 year following up, the result showed that spasticity was reduced in all cases and the function of extremities was improved significantly in some cases. Conclusion: Selective nerve root rhizotomy, especially partial C 8 rhizotomy, is effective in reducing spasticity of the extremities and in improving the function of the hand.
