儿童鞍区生殖细胞瘤

目的：探讨儿童鞍区生殖细胞瘤的诊断和治疗。方法：回顾性分析我科手术治疗的儿童鞍区生殖细胞瘤23例的临床和随访结果。结果：显微镜下全切除7例，次全切除15例，部分切除1例，术后死亡1例，死亡率4．35％。21例术后行放疗，其中19例行全脑加脊髓轴放疗。平均随访3．7年，复发率17．6％，种植播散2例，发生率11．7％。结论：儿童鞍区生殖细胞瘤常以尿崩为首发临床表现，应尽可能行肿瘤全切除术。综合治疗可望获得满意的疗效。

Chidren Germinomas in Sellar Region

Objective:To investigate the diagnosis and treatment of children germinomas in sellar region.Methods:Retrospective analysis was performed on the data of 23 cases of with germinomas in Sellar region in our department.Results:Under the operative microscope,the tumors were totally removed in 7 cases,subtotally in 15 Cases,and partially in 1 cases.1 case died after operation(4.35%).21 cases were performed post-operative radiotherapy,and 19 cases with whole brain-spinal axion radio therapy.Average fellow up was 3.7 years,3 cases were recurred after treatment (17.6%) and 2 cases were implant metastasis(11.7%).Conclusions:Diabetes is usually the initial manifestation of children germinoma in sellar region.The tumor should be totally excised as possible.Satisfied curative effect can be gained by combined treatment.
