摘    要:目的 观察膈肌下抬挤心肺复苏(D-CPR)方法对心搏骤停猪循环和通气的影响.方法 健康家猪8头,窒息法建立猪心跳骤停模型后行膈肌下抬挤法复苏,测定并比较8头猪在基础状态下和心搏呼吸停止行膈肌下抬挤复苏时的收缩压、舒张压、右房压和潮气量值(VT),计算每分通气量和冠脉灌注压;记录自主循环恢复时间(ROSCT)和6 h后存活率.结果 膈肌下抬挤心肺复苏法(D-CPR)产生的冠脉灌注压和潮气量达到基础值30%左右,与基础状态相比,差异有统计学意义(P〈0.05);6 h存活率为100%.结论 在窒息性心搏骤停猪模型中,膈肌下抬挤能产生有效的冠脉灌注压和少量肺通气.

Abstract:Objective To observe the effect of cardiopulmonary resuscitation by cardiac massage under the diaphragmatic muscle (D-CPR)to pig's circulation and ventilation at cardiac arrest. Methods Eight healthy pigs were induced cardiac arrest by being given over dosed bupivacaine as the hemodynamic was stable for 20 minutes after being anesthetized. Their systolic blood pressure (SBP), diastolic blood pressure (DBP), central venous pressure (CVP) and tidal volume (VT) were recorded as action of resuscitation of D-CPR was taken, and coronary perfusion pressure (CPP) and minute ventilation (MV) were calculated. Results Both of CPP and VT of D-CPR during performing cardiac massage exceeded 30% over the basic level. Conclusion D-CPR may provide effective coronary perfusion pressure and a small quantity of ventilation in cardiac arrest.
