梗阻性结直肠癌的诊治分析

目的：总结梗阻性结直肠癌的诊治经验。方法：回顾分析１９８０～２０００年收治９４例梗阻性结直肠癌病人的影像学（腹部平片、钡灌肠、结肠镜、ＣＴ）表现和治疗方法。全组一期切除吻合５１例（５４．３％），分期手术８例，转流捷径手术８例，Ｈａｒｔｍａｎｎ手术３例， Ｍｉｌｅｓ手术６例，单纯结肠造口１６例，单纯探查术２例。结果：住院期间死亡３例，吻合口漏一例，９０例（９５．７％）恢复顺利，康复出院。结论：应根据梗阻性结直肠癌病人全身情况和局部条件以及医疗单位技术水平实施治疗方案。

Analysis of the Diagnosis and Treatment in Colorectal Cancer with Obstruction.

Objective: To sum up the experience of diagnosis and treatment in obstructive colorectal cancer. Me- thods: The imaging manifestation and therapeutic strategies in 94 cases of obstructive colorectal cancer from 1980 to 2000 were analysed retrospectively. Of which, 51 cases underwent primary resection and anastomoses (54.3%), 8 cases to multi-stage operation 8 cases to by-pass procedures, 3 cases to Hartmann's procedure, 6 ca- ses to Mile's operation, 16 cases to simple colostomy and 2 cases to simple laparotomy. Results: Post-operative death occurred in 3 cases, anastomotic leakage in 1 ease, and 90 of 94 cases (95.7%) were recovered satis- factorily. Conclusions: The therapeutic strategy should be individulized according to the general condition of the patient, the local conditions of the lesion and the technical skills of the staff.
