腹腔镜下采用卵巢良性肿瘤剥除对生殖内分泌功能的影响研究

目的：探讨腹腔镜下卵巢良性肿瘤剥除对生殖内分泌功能的影响。方法选取2011年9月～2013年3月来我院就诊的卵巢良性肿瘤患者127例，根据治疗方法分为观察组（79例）及对照组（48例）。对照组采用常规经腹手术，观察组采用腹腔镜进行手术。观察两组患者治疗前后LH、E2、FSH各指标及手术时间、术中出血量、术后下床活动时间及术后住院时间等指标。结果观察组患者治疗前后比较，LH、E2、FSH各指标差异均显著（t=35.136，9.354，12.312，P＜0.05）。对照组患者治疗前后比较，LH、E2、FSH各指标差异均显著（t=28.651，16.357，4.137，P＜0.05）。观察组治疗后LH、E2、FSH各指标与对照组比较，差异有统计学意义（t=29.316，13.168，9.662，P＜0.05）。观察组手术时间、术中出血量、术后下床活动时间及术后住院时间均优于对照组（t分别为7.356，21.245，6.346，9.116，P＜0.05）。观察组的AMH指数，基础窦卵泡数（AFC）及bFSH等指标皆显著高于对照组，差异显著（t分别为6.337，7.365，5.134，P＜0.05）。结论卵巢良性肿瘤剥除具有较好的安全性及可靠性，可以较好的保留患者的生殖内分泌功能。

Study on effect of laparoscopic benign ovarian tumor resection on reproductive endocrine function

Objective To explore the effect of laparoscopic benign ovarian tumor resection on reproductive endocrine function. Methods 127 patients with benign ovarian tumors from September 2011 to March 2013 in our hospital were selected,and divided into the observation group（79 cases） and control group（48 cases） according to the treatment methods.The control group used conventional transabdominal operation,the observation group was treated with laparoscopic operation.Two groups were observed before and after treatment in patients with LH,E2,FSH indexes and operation time,amount of bleeding,postoperative ambulation time, postoperative hospitalization time. Results The differences were significant in the indexes of LH,E2,FSH in the observation group（t=35.136,9.354,12.312, P 〈 0.05）and control group（t=28.651,16.357,4.137,P 〈 0.05）between before and after treatment.Compared with the control group after treatment,the indexes of LH,E2,FSH in the observation group after treatment had significant difference（t=29.316,13.168,9.662,P 〈 0.05）.The operation time,amount of bleeding,postoperative ambulation time,postoperative hospitalization time in the observation group were better than those in the control group（t=7.356,21.245,6.346,9.116,P 〈 0.05）,the AMH index,AFC and bFSH were significantly higher than those in the control group（t=6.337,7.365,5.134,P 〈 0.05）. Conclusion Benign ovarian tumor resection has safety and reliability,as well has a good effect on the preservation of reproductive endocrine function.
