回顾性分析ERCP影像对临床的意义

目的探讨内窥镜逆行胰胆管造影引导行oddis括约肌切开套取胆总管结石的应用价值。方法对我院近三年来42例阻塞性黄疸患者在DSA引导下行ERCP＋EST治疗的临床资料的进行回顾性分析。结果 ERCP确诊42例均为阻塞性黄疸,其中胆总管结石33例,Mirizzi综合征4例,壶腹肿瘤5例。其中31例oddis括约肌切开套取结石,3例在DSA引导下行支架扩张术解除黄疸。其余8例除1例放弃治疗外均中转手术治疗。结论 ERCP是诊断阻塞性黄疸确切有效的方法,EST和气囊扩张狭窄胆管置入支架解除黄疸简便,成功率高,为安全有效的治疗的方法,为患者安全度过围手术期行手术治疗创造了条件。ERCP不仅可直观胆总管病并的部位采取治疗外,还可进行活检及细胞学检查,是目前公认的诊断胰胆管疾病的首选方法。

Retrospective Analysis of Endoscopic Retrograde Cholangiopancreatography X-ray Image of the Clinical Significance

Objective To investigate the effect of endoscopic retrograde cholangiopancreatography sphincterotomy guided Oddis extract common bile duct stones application value.Methods The hospital for nearly three years,42 cases of patients with obstructive jaundice under guidance of DSA ERCP＋EST in the treatment of clinical data were retrospectively analyzed.Results ERCP diagnosis in 42 cases of obstructive jaundice,of which 33 cases of common bile duct stones in 4 cases,Caroli 5 cases,tumor of the ampulla of vater.Among the 31 cases of Oddis sphincterotomy obtain calculi,3 cases in DSA guided stent dilatation relieve jaundice.The remaining 8 patients except 1 case abandoned treatment are transit operation treatment.Conclusion ERCP is the diagnosis of obstructive jaundice effective method,EST and balloon dilatation of stricture of bile duct stenting relieves jaundice is simple,high success rate,safe and effective treatment method,for patient safety through operation treated with surgical operation to create the conditions.ERCP can not only direct bile duct disease and parts take treatment,can also carry out biopsy and cytology,is now recognized as the preferred method for the diagnosis of pancreaticobiliary duct diseases.
