糖尿病视网膜病变血清IgG型抗内皮细胞抗体检测

摘    要:目的:探讨血清IgG型抗内皮细胞抗体(AECA)的阳性表达及其在不同分型糖尿病视网膜病变(DR)间的变化趋势。方法:根据荧光素眼底血管造影(FFA)将糖尿病视网膜病变患者分为增殖型糖尿病视网膜病变(PDR)组、背景型糖尿病视网膜病变(BDR)组、糖尿病无视网膜病变(NDR)组及正常对照组。用体外培养的人脐静脉内皮细胞(HUVEC)作为抗原,采用间接免疫荧光(IIF)法检测不同分型患者血清IgG型抗内皮细胞抗体阳性率的变化,并与正常对照组进行对比。结果:血清IgG型AECA阳性表达BDR组和PDR组均高于NDR组和正常对照组(P〈0.05),而BDR组和PDR组间阳性率差别无统计学意义(P〉0.05),NDR组和正常对照组间阳性率差别无统计学意义(P〉0.05)。结论:随着由正常眼底向DR的转变,血清IgG型AECA阳性率呈增高趋势,表明AECA与DR存在内在关联。

Abstract:Objective: To detect the existence of IgG anti-endothelial cell antibodies(AECA) in serums by indirect immunoflorescence (IIF) and discuss the trend of the change in different types of diabetic retinopathy(DR). Methods: DR patients were divided into proliferative diabetic retinopathy(PDR) group, background diabetic retinopathy(BDR) group, non-diabetic retinopathy(NDR) group and the control group by fundus fluorescein angiography(FFA). Human umbilical vein endothelial cells(HUVEC) cultured in vitro were used as antigen, the variation of positive rates of the IgG AECA in the serums of different groups were detected by IIF, and compared with the control group. Results: In the comparison of positive rates of IgG AECA, the rates of BDR group(80%) and PDR group(85%) were higher than those of NDR group (5%) and control group (5%) (P < 0.05). There was no significant difference of positive rate between BDR group and PDR group (P < 0.05), and there was no significant difference of positive rate between NDR group and control group (P > 0.05). Conclusion: With the change from normal retina to DR, the positive rate of IgG AECA in serums gradually increases, which means that AECA has inherent relationship with DR.
