急性出血坏死性胰腺炎30例治疗体会

本文报告急性出血坏死性胰腺炎３０例，由科综合治疗３例，外科手术治疗２７例，死亡８例，病死率２６．７５。作者认为，合并急性胆囊炎、胆石症，腹腔或胰腺周围并发化性感染，腹膜刺激症状和严重的酶性毒血症时，内科综合治疗无效为手术适应证。讨论了手术方式，介绍了坏死组织清除，应用Ｕ形双头双套管和多方位双套管引流灌洗的体会。

Experience with Treatment of 30 Patients with Acute Heamorrhagic Necrotizing Pancreatitis

From 1980 through 1993 30 patients with acute heamorrhagic negrotizing pancretitis(AHNP) were tre ?ated in our department. Of these patients,3 cases received medical therapy,other twenty seven cases under went surgical operation,eight patients died with mortality 26. 7% (8/30). Analysis of this series shown that the indications for surgical operation were following: AHNP with acute cholecystitis and cholelithiasis; complicated by purulent infection around the pancreas or in the peritoneal cavity; with obvious peritoneal irritation or severe toximia from pancreatic enzymes;not responding to medical complex trentment. The operative methods were discussed.
