硬膜外阻滞麻醉用于无痛分娩的临床观察

目的:探讨硬膜外阻滞麻醉用于无痛分娩的疗效和对产程进展及母婴状况的影响。方法:选择自愿要求分娩镇痛的单胎头位、无麻醉禁忌证的初产妇250例,宫口开大(3～4)cm时,行PECA泵持续硬膜外腔给药分娩镇痛(观察组),并与同期条件相似、未给予任何镇痛方法的250例初产妇进行对照(对照组)了解观察组镇痛效果、不良反应,及对产程、分娩方式、产后出血、胎儿及新生儿的影响。结果:观察组和对照组产程比较、两组分娩方式比较有显著性差异(P<0.05);两组胎儿宫内窘迫,新生儿窒息及产后出血发生率比较,无显著性差异(P>0.05)。结论:PECA泵用于持续硬膜外腔给药镇痛,疼痛阻滞完善,可达到分娩镇痛效果,加速产程,降低了剖宫产率、阴道难产率,对母婴均无不良影响。

The Clinical Observation of Painless Delivery by Using Epidural Anesthesia

Objective:To discuss the clinical observation of painless delivery by using epidural anesthesia and curative effect of progress and maternal status.Methods:Choose voluntary requirements labor analgesia single fetal head bits,without anesthesia contraindication of primigravida 250 example,palace mouth open big(3～4)cm,line PECA pump continuous epidural cavity dosing labor analgesia(the observation group),and compared with the similar conditions,not give any demulcent method of 250 example primigravida comparison(control group) understand the observation group analgesic effect,adverse reactions.and labor,delivery modes,postpartum hemorrhage,fetus and neonatal influence.Results:Trial group and control group were compared,two groups delivery method with significant difference(P<0.05).Compared two groups of neonatal asphyxia intrauterine fetal distress,there was no significance difference in incidence of postpartum hemorrhage(P>0.05), Conclusion:PECA pump used for continuous epidural analgesia,pain cavity dosing block perfect,can achieve labor analgesia effect,accelerate labor,reduced cesarean section rate,vaginal dystopia rate,there were no bad effect to maternal and child.
