文拉法辛联合认知疗法治疗强迫症对照研究

目的探讨文拉法辛联合认知疗法治疗强迫症的临床疗效。方法将84例强迫症患者随机分为两组，每组42例，两组均口服文拉法辛治疗，研究组在此基础上联合认知治疗，观察4个月。于治疗前及治疗1个月、2．5个月、4个月末采用Yale-Brown强迫量表评定临床疗效。结果治疗后两组Yale-Brown强迫量表评分均较治疗前显著下降（P〈0．01），但同期研究组均较对照组下降显著（P〈0．05或0．01）；治疗4个月末，研究组显效率52．4％、有效率88．1％，对照组分别为42．9％、73．8％；研究组显效率、有效率均高于对照组，但差异无显著性（χ2=0．764、2．779，P〉0．05）。结论文拉法辛治疗强迫症疗效显著，但在此基础上联合认知领悟治疗起效更快，疗效更显著。

A control study of venlafaxine combined with cognitive therapy in the treatment of obsessive-compulsive disorder

Objective To explore the chinical efficacy of venlafaxine plus cognitive therapy in the treatment of obsessive-compulsive disorder （OCD）. Methods Eghity-four OCD patients were randomly divided into two groups of 42 ones each, both groups took orally venlafaxine, research group was plus congnitive therapy for 4 months. Clinical efficacies were assessed with the Yale- Brown Obsessive-compulsive Scale （Y-BOCS） before treatment and at the end of month1, 2.5 and 4. Results After treatment, the scores of the Y-BOCS of both groups lowered more significantly compared with pretreatment （P〉0.01） , so did in the research group than in the control group according to contemporary comparison （P〈0. 05 or 0. 01）; both obvious effective and effective rates were higher in the research group than in the control group （52, 4% vs. 42.9%, 88.1% vs. 73.8%）, but differnces had no signifi- cance （χ2=0. 764.2. 779,P〉0.05）. Conclusion Venlafaxine has a significant curative effect in OCD, but its combination with congnitive therapy takes effect more rapidly and is more effective.
