老年患者妇科腹腔镜手术临床研究

目的 研究老年患者妇科腹腔镜手术的临床特点，评价腹腔镜手术在老年患者妇科疾病中应用的安全性及价值。方法 1997年1月～2004年10月该院70例60岁以上老年妇科疾病患者行腹腔镜手术治疗，对临床资料进行回顾性分析。结果 该组78．57％（55／70）患者合并有不同程度的心血管疾病、糖尿病及肺脏疾病，手术指征包括盆腔、子宫、卵巢和输卵管等各种良性疾惠以及早期恶性肿瘤。95．71％（67／70）镜下手术成功。并发症发生率7．46％（5／67），全组病例无电损伤及手术死亡。结论 做好围手术期处理，腹腔镜手术可安全地应用于多种老年妇科疾病的治疗，并可在不断提高腹腔镜技术水平的基础上逐步扩大手术适应证。

Clinical study on characteristics of laparoscopy on elderly patients with gynecopathia

Objective] To investigate the clinical characteristics of laparoscopy on elderly patients with gynecopathia. Methods] Between 1997 and 2004, 70 patients(all over 60 years old) with gynecopathia underwent laparoscopy in our department. All the data were analysized retrospectively. Results] Of the 70 patients, 55 women(78.57%) also suffered from diabetes , cardiovascular diseases and/ or bronchitis. Innocence disease of pelvic cavity and gynecological malignant tumor in earlier period were included in the group. After the operation, 67 patients(95.71%) recovered successfully, and only 5 cases(7.46%) had operation-related complications. No electrical injury or death cases reported. Conclusions] With good perioperation treatment, laparoscopy is an optimal treatment for elderly patients with gynecopathia. We can also enlarge the operative indication by sprinting to advanced laparoscopic techniques.
