中药冠心一号方治疗冠心病48例临床研究

【目的】观察具有益气养阴、活血祛瘀作用的中药冠心一号方(由党参、麦冬、五味子、玉竹、川芎、知母、丹参、三七、郁金组成)治疗冠心病心绞痛的疗效及其对血脂、血液流变学指标的影响。【方法】采用随机数字表法将88例患者随机分为治疗组(48例)和对照组(40例)。两组均给予西药消心痛、倍他乐克和肠溶阿斯匹林治疗,治疗组在此基础上服用中药冠心一号方治疗,两组均以4周为1个疗程。观察两组的临床症状总疗效、心绞痛疗效和心电图疗效,并于治疗前后检测两组患者的血脂水平和血液流变学指标。【结果】治疗后治疗组的临床症状总疗效、症状评分、心绞痛疗效和心电图疗效均优于对照组(P<0．05或P<0．01)。治疗后治疗组血中总胆固醇(TC)、甘油三酯(TG)含量均有下降(P< 0．05),而对照组下降不明显(P>0．05)。治疗后两组的全血高切粘度(WBHSV)、全血低切粘度(WBLSV)均有下降 (P<0．05或P<0．01),且治疗组比对照组下降更明显(P<0．05);治疗组的血浆粘度(PV)、红细胞聚集指数均有下降 (P<0．01),而对照组的下降作用不明显(P>0．05)。【结论】中药冠心一号方能较好地改善冠心病心绞痛患者的临床症状及心电图缺血表现,这可能与其降低患者的血脂水平和改善血液流变学指标有关。

Effect of Guanxin No. 1 Recipe for Coronary Heart Disease: An Observation of 48 Cases

Objective] To investigate the therapeutic effect of Guanxin No. 1 Recipe (mainly composed of Radix Codonopsis, Radix Ophiopogonis, Fructus Schisandrae Chinensis, Rhizoma Polygonati Odorati, Rhizoma Chuanxiong, Rhizoma Anemarrhenae, Radix Salviae Miltiorrhizae, Radix Notoginseng, Radix Curcumae) for coronary heart disease (CHD) with angina pectoris (AP) and to observe its influence on blood lipid and hemorrheology indexes. Methods] Eighty-eight patients with AP were allocated to group A (n = 48) and group B (n = 40) according to random number table. The two groups were given isosorbide dinitrate, Metoprolol Tartrate Tablets and enteric-coated aspirin tablets, and group A was treated with Guanxin No. 1 Recipe additionally. Four weeks constituted one treatment course. The effects on TCM syndromes, AP, electrocardiogram (ECG) and symptoms scoring, as well as on blood lipid level and hemorrheology indexes were observed before and after treatment. Results] After treatment, the effects on TCM syndromes, AP, electrocardiogram and symptoms scoring in group A were superior to those in group B (P < 0.01 or P < 0.05 compared with group B) ; blood levels of total cholesterol (TC) and triglyceride (TG) were decreased in group A (P < 0.05) but the decrease was not obvious in group B (P > 0.05) . Whole blood high-shear viscosity (WBHSV) and whole blood low-shear viscosity (WBLSV) were decreased in the two groups (P < 0.01 or P < 0.05) and the decrease in group A was obvious as compared with that of group B (P < 0.05); plasma viscosity (PV) and erythrocyte aggregation index were also decreased in group A (P<0.01) but the decrease was not obvious in group B (P >0.05). Conclusion] Guanxin No. 1 Recipe has a better effect on relieving symptoms and improving EGG in CHD with AP, and this may be related to the decrease of blood lipid level and improvement of hemorrheology index.
