海马硬化的扩散张量成像参数值变化初步研究

【目的】探讨海马硬化（HS）患者的扩散张量成像（DTI）参数值变化特点，初步评价DTI在海马硬化术前诊断中的价值。【方法】对11例单侧海马硬化患者和32例健康志愿者行常规MRI和DTI检查，定量测量双侧海马的平均扩散系数（DCavg）、部分各向异性（FA）、1-容积比（1-VR）和相对各向异性（RA）值并统计。【结果】常规MR1：7例左侧海马硬化，4例右侧海马硬化，无双侧海马萎缩、硬化。正常人双侧海马之间DCavg、FA、1-VR和RA均无显著性差异。海马硬化患者：患侧海马DCavg显著高于对侧海马和正常人，患侧海马FA、1-VR和RA低于对侧海马和正常人。【结论】海马硬化的DTI参数值变化具有一定特征性，DCavg升高、FA、1-VR、RA减低能够作为诊断海马硬化的有用指标。

Preliminary study on diffusion tensor imaging parameters in hippocampal sclerosis

To investigate the diffusion characteristics of hippocampi in patients with hippocampal sclerosis and to study the value of diffusion tensor imaging （DTI） in preoperative diagnosisof hippocampal sclerosis. [Methods] Eleven patients of unilateral hippocampal sclerosis and 32 healthy volunteers received conventional and DTI scanning. The DCavg, FA, 1-VR and RA of bilateral hippocampi were measured respectively. [Results] On conventional images, 7 of 11 cases had fight hippocampal pathology, and 4 of 11 cases had left hippocampal pathology. There were no bilateral hippocampal atrophy. No significant differences of DCavg, FA, 1-VR and RA were noted between right and left hippocampi in the controls.The DCavg of ipsilateral hippecampi was higher than contralateral side and healthy controls. The FA, 1-VR, RA of ipsilateral hippocampi were lower than contralateral side and healthy controls. [Conclusions] DTI parameters of hippocampal sclerosis was characteristic and the increased DCavg, decreased FA, 1-VR and RA could be used as an useful index of diagnosing hippocampal sclerosis.
